BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - REACTOR BUILDING DOORS DEFICIENT WELDS BY JOHNSON MACHINE WORKS - BLRD-50-438/82-58, BLRD-50-439/82 FINAL REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. V. Crlenjak on August 16, 1982 in accordance with 10 CFR 50.55(e) as NCR 1929. This was followed by our first interim report dated September 13, SS2, Enclosed is our final report. We consider 10 CFR l Part 21 applicable to this deficiency.

Descrintion of Deficiency TWo previous nonconformance reports (NCRs 1463 and 1638) documented deficiencies on equipment supplied by Johnson Nachine Works, Chariton, Iow a.

NCR 1929 was initiated to document additional deficiencies on equipment supplied by Johnson Machine Works as well as the breakdown in investigating the generic implications in accordance with response to violation 50-438, 50-439/ 82-23-04' Inadequate Investigation of Generic Aspects and Failure to Identify Cause for Significant Deficiencies.' TVA inspected all Johnson -

Nachine Works- supplied items at Bellefonte, and found that the Reactor Building (RB) equipment hatch and Diesel Generator Building doors were to have been f abricated to American Welding Society (hts) section D1.1 standards . The welds on the doors do not conform to the AWS D1.1 standards.

i The cause for deficient vendor welds may be credited to a potential vendor QA breakdown caused by poor quality workmanship and f ailure to define hold points for these welds in appropriate drawings / specifications and the f ailure of the TVA inspector to recognize the weld deficiencies during inspections performed at the vendor's shop on these items.

The cause of TVA's f ailure to identify generic implications was addressed in the response to Violation 50-438, 50-439/82-23-04.

Safety Innlications The cited RB doors are part of the Secondary Containment pressure boundary. Should the subject welds f ail, there is a possibility that the Secondary Containment pressure boundary could be compromised which

excess of 10 CFR Part 100 guidelines. Failure of the subj ect welds on the .

Diesel Generator Building doors would not compromise the ability of the doors to perform their safety function.

Corrective Action TVA will rework the Reactor Building equipment hatch doors to meet requirements shown on the vendor's drawings. The indicated corrective action will be complete by August 3,1983. The Diesel Generator Building missile doors are acceptable as fabricated and will be used

'as is.'

The actions required to prevent recurrence of the similar deficiencies documented. on NCR 1638 and delineated in our second revised final response on that deficiency should also preclude recurrence of the deficiencies l dispositioned by this report.

s4 The actions prescribed in the second revised final response on NCR 1638 documented a revision to the TVA Quality Engineering Branch Administrative Instruction (QEB-AI 313.1) , ' Training and Certification of QEB Personnel,'

which will require American Welding Society certification for QEB source inspectors. IVA's QEB also committed to allow frore inspector time for the inspection of safety-related components.

. The corrective action taken in response to Violation 50-438,50-439/82-23-04 and the implementation of the G. H. Kimmons' memorandum to M. N. Sprouse and H. H. Null dated July 21, 1982, ' Identification of Root Cause and Generic Implications for Significant Conditions Adverse to Quality,' (EDC 820721 001), should prevent recurrence of TVA's f ailure to adequately evaluate the generic implications of documented nonconformances.
