DUKE POWER COMPANY REPLY TO NOTICE OF VIOLATION 413/91-11-01 Technical Specification 6.8.1 requires in part that written procedures be established, implemented, and maintained covering the operation of the onsite emergency power sources.
Operations Management Procedure 2-18, Tagout Removal And Restoration (R&R) Procedure, Section 6.3, Rostoration Entries, Step 6.3.B requires in part that when a tagout (R&R) is being restored, sequence numbers be assigned indicating the order in which the equipment is to be returned to service.
Contrary to the above, on April 12, 1991, during the process of clearing Diesel Generator 1A block tagout, R&R #11-461, operations personnel failed to complete the assignment of sequence numbers for the restoration of the equipment and failed to realign the equipment, as specified by the partial assignment of sequence numbers, resulting in a spill of approximately 1500 gallons of diesel fuel oil in the 1A Diesel Generator room.
RESPONSE
1.
Reasons for Violation if Admitted This incident is attributed to an inappropriate action because the Non-Licensed Operator (NLO) did not apply a sufficient level of attention to the R&R restoration sequence; the three valves designated Sequence Number 1 were overlooked and consequently not positioned prior to realigning other Block Tagout valves.
2.
Corrective Actions Taken and Results Achieved The NLO closed valve 1FD-047, terminating the fuel oil spill.
The spilled fuel oil was pumped to the Used Lube Oil Storage Tank.
The appropriate notifications were made concerning the spill.
The incident was discussed with those involved.

procedures, to prevent components being placed in other-than-desired positions, was communicated to all Operations shifts through special meetings with the Station Manager.

Page 2 been formed to evaluate these incidents and develop recommendations as needed.
Supervision met with each operations shift and discussed this incident and the importance of following the sequence on R&Rs.
An Operator Update was issued, emphasizing the need for correct implementation of each step in the R&R process.
This incident was evaluated regarding environmental complitnce, and it was determined that there was no enviror. mental impact.

Corrective Actions to be Taken to Avoid Further Violations Actions taken in Section 2 above ensure avoidance of further violations.

Date of Full Compliance Duke Power is now in full compliance.
