Appendix NOTICE OF VIOLATION Monsanto Research Corporation License No. SNM-567 As a result of the inspection conducted on August 15-19, 1983, and in accordance with the NRC Enforcement-Policy, 47 FR 9987 (March 9, 1982), the following violations were identified:
- 1. License Condition No. 16 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated June 16, 1977; and various dated letters with attachments thereto.
The'above referenced application, Item No. 14, references Section 6.0 of your Operations Manual. Section 6.0 of the Operations Manual contains Subsection 6.2 entitled " Radiation Monitoring and Contamination Control."
Subsection 6.2.2 (1) states "As a matter of routine, operating personnel are required to conduct appropriate radiation surveys prior to, during, and at completion of individual work assignment involving the handling or processing of radioactive material."
. Contrary to this requirement, an individual handling radioactive material failed to perform an appropriate survey to detect contamination on himself and'his clothing prior to leaving your controlled area.
Specifically, on April 25, 1983, a Health Physics Technician working '
with americium-241 failed to properly survey himself prior to leaving the plant for lunch. It was later discovered that the individual had contamination on his right forearm and clothing.
This is a Severity Level IV violation (Supplement VI).

. Appendix 2 and two boxes in Room No. 9 did not contain fire extinguishers. These glove boxes are currently used for processing radioactive materials in normal form.
This is a Severity Level IV violation (Supplement VI).
With respect to item 1, the inspection showed that action had been taken to correct the identified item of noncompliance and to prevent recurrence.
Consequently, no reply to this item of noncompliance is required and we have no further questions regarding this matter. With respect to item 2, pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for this item of noncompliance: (1) corrective action taken and the results achieved; (2) corrective action to be -taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
