The subject deficiency was initially reported to NRC-0IE Inspector R. V. Crlenjak on October 21, 1981 in accordance with 10 CFR 50.55(e) as NCRs BLN NEB 8113 through BLN NEB 8118. This was followed by our first interim report dated November 16, 1981..Related NCRs BLN NEB 8119 R2, 8120, and 8205 and BLN NES 8203 R2 were. reported in our second interim report dated May 10, 1982,' and NCRs BLN NEB 8208, 8209, 8211, 8213, 8214, and 8215, and GEN QAB 8204 were reported in our third interim report dated August 16, 1982. Since that i

time related NCRs BLN NEB 8217 and 8218 and BLN NEB 8203.R2, 8303, 8304, 8310 8311, 8312 8313, 8314, and 8315 have also been reported. Enclosed is our fourth

interim report. We expect to submit our next report by July 20, 1984.

FOURTH INTERIM REPORT Description of Deficiency During TVA's NUREG-0588 environmental qualification program, the component listed in Table I - Parts A and B (attached) were identified as potentially deficient because of insufficient documentation to verify that the equipment is environmentally qualified. This equipment was procured before the issuance of NUREG-0588.

Interim Proeress Since TVA's previous report, fourteen additional nonconformance reports (NCRs) have been generated to identify equipment potentially deficient because of insufficient documentation. Two of these NCRs, BLNEEB8313 and BLNEEB8314 were written against generic items (terminal blocks and handswitches) which are located in the turbine-driven auxiliary feedwater pump rooms and the main steam valve vault rooms. TVA is in the process of identifying the specific items which are generically identifed by these two NCRs. The remainder of the additional NCRs are included in the attached table.

Also included in the attached table are those NCRs which have been previously reported on and for which TVA has either changed the corrective action previously reported or completed their review of the equipment identified.
