Friday, 6/5/81 ,,

, 0900(approx) Routine operator tour finds chabs and locks removed from Auxiliary Feedwater Pump local, manual suction valves. Valves found in proper (open) position.

DLC investigation. Valves immediately relocked. Considered as possible operator oversight.

1630(approx) Auxiliary operator observes HHSI common suction valve (SI-26) open. Does not recall if valve handwheel was chained and locked 1630-1645 Same auxiliary operator participates in SI accumulator fill .

(approx) from same building area as contains SI-26. Accumulator fill flowpath is through SI-26. Added about 45 gallons to accumulator from RWST.

4 -Saturday, 6/6/81 0100 Auxiliary Operator finds SI-26 shut, chain and padlock missing; notifies control room. SR0 dispatched to scene, valve reopened

and relocked.

4 0115 Shift Supervisor notifies station Operations Supervisor at home.

Operators initiate valve alignment checks for all ESF manual valves.

0120 Security force notified by Shift Supervisor and is directed to isolate Auxiliary Building.

0140 HQ:D0 and SRI notified by Shift Supervisor l 0145 Shift Supervisor implements "two man rule" for access to auxiliary building.

, 0215(approx) Doors to auxiliary building, switchgear rooms, EDGs locked out on computer; guard posted'at auxiliary building door to restrict

-access to operations and security personnel only.

[;g SEQUETCE OF EVENTS 0630 (approx) - SRI arrives onsite, briefed by Shift Supervisor, reviews 0800 plant status and licensee response. ..

0800 (est) SRI informs HQ;D0 that he is onsite for followup and will contact RI management 0820 (approx) SRI briefs RPS No. 2A Section Chief 0840 (approx) SRI briefs Security Section Chief Licensee management arrives onsite & begin review. SRI advises 0900 - 1030 (approx) licensee of need for' prompt FBI notification and licensee followup actions. SRI has further update telecons with RI management 1030-1130 SRI offsite 1130-1330 Additional SRI discussions with RI management and licensee managemen re licensee need to notify FBI.

SRI reviews licensee actions / verifies completion of valve lineup checks, establishment of additional security tours, implementation of two man rule, lockout of vital area doors, performarice of supplemental operator tours.

1330 (approx) Licensee notifies FBI. SRI informs RI management of notification.

1530(approx) FBI arrives onsite.

1530-2030 SRI and licensee management brief agents, escort to valve locations. Agents collect possible physical evidence.

DLC and FBI review licensee search and security measures; I SRI monitors.

Sunday, 6/7/81 0830-0930 SRI arrives onsite. Confirms plant and security status, (approx) reviews logs and receives update br.iefing from Shift Super-visor.

1000-1300 SRI tours plant with Shift Supervisor. Verifies manual locked (approx) ESF valves properly positioned, emergency bus MCCs properly aligned, selected instrumentation valved in for ECCS and AFW systems, visually checked containment isolation valves and electrical penetration integrity, visually checked main control board instrumentation and equipment status lites, inspected cable tunnels, switchgear rooms, auxiliary building for abnormal conditions, verified security measures implemented in auxiliary building. Reviewed scope of licensee equipment verifications with plant operators.
