, 0900(approx) Routine operator tour finds chabs and locks removed from Auxiliary Feedwater Pump local, manual suction valves. Valves found in proper (open) position.
DLC investigation. Valves immediately relocked. Considered as possible operator oversight.
1630(approx) Auxiliary operator observes HHSI common suction valve (SI-26) open. Does not recall if valve handwheel was chained and locked 1630-1645 Same auxiliary operator participates in SI accumulator fill .
(approx) from same building area as contains SI-26. Accumulator fill flowpath is through SI-26. Added about 45 gallons to accumulator from RWST.
4 -Saturday, 6/6/81 0100 Auxiliary Operator finds SI-26 shut, chain and padlock missing; notifies control room. SR0 dispatched to scene, valve reopened

SRI advises 0900 - 1030 (approx) licensee of need for' prompt FBI notification and licensee followup actions. SRI reviews licensee actions / verifies completion of valve lineup checks, establishment of additional security tours, implementation of two man rule, lockout of vital area doors, performarice of supplemental operator tours.

1000-1300 SRI tours plant with Shift Supervisor. Verifies manual locked (approx) ESF valves properly positioned, emergency bus MCCs properly aligned, selected instrumentation valved in for ECCS and AFW systems, visually checked containment isolation valves and electrical penetration integrity, visually checked main control board instrumentation and equipment status lites, inspected cable tunnels, switchgear rooms, auxiliary building for abnormal conditions, verified security measures implemented in auxiliary building. Reviewed scope of licensee equipment verifications with plant operators.
