Watson on June 14, 1983 in accordance with 10 CFR 50.55(e) as NCRs BLN 2383, 2384, 2385, and 2395. Enclosed is our first interim report. FIRST INTERIM REPORT Description of Deficiency NCHs 2383, 2384, and 2385 identify that in several electrical cable penetration terminations the correct application of Raychem tubing, end caps, or cable breakouts was not adhered to per TVA drawings. This has resulted in incorrect lengths of Raychem tubing on cable splices, insufficient distance between Raychem 2-and 3-point cable breakouts, and no end caps on spare conductor.a.
NCR 2395 was issued to identify the 6eneric implication of the three previously mentioned NCRs and a possible generic deficiency in the electrical quality control (EQC) training and inspection program. The apparent cause of these deficiencies is a lack of proper interpretation of TVA drawings by TVA Construction (CONST) craft and EQC inspectors.
Interim Progress NCRs 2383 and 2384 address the inadequate lengths of Raychem tubing. TVA has reinspected the affected cable penetration tersimations and is presently investigating the possibility of using the terminations "as is."
NCR 2385 addresses the lack of Raychem end caps on spare cable penetrations. TVA's Division of Engineering Design (EN DES) is reviewing the recommendation that a work release be generated to check all previous terminations and install end caps on spare conductors as required per applicable TVA drawings. TVA CONST craft personnel and EQC inspectors will be retrained, per Bellefonte Nuclear Plant (BLN) Quality Control Procedure (QCF) 10.29, in the proper application of termination procedures per TVA drawings.
