APPENDIX A NOTICE OF VIOLATION Duke Power Company Docket Nos. 50-369 and 50-370 McGuire 1/2 License Nos. NPF-9 and NPF-17 As a result of the inspection conducted on May 20 - June 20, 1983, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violation was identified.
Technical Specification 6.8.1.a requires that written procedures be established, implemented and maintained covering surveillance testing and maintenance.
Contrary to the above, the licensee technicians and operations personnel fa41ed to abide by the Technical Specification requirement in that:
a) on Jine 11, 1983, at 12:54 p.m., an electronic technician failed to follow p?ocedure IP-0-A-3010-05 during the performance of Unit 2 S!'s functional test, resulting in actuation of an improper relay which caused a reactor trip. b) on June 2, 1982, maintenance technicians failed to abide by the requirements of Station Directive 3.1.19, when they failed to have the Unit 1 B Boric Acid Transfer pump isolated and logged prior to disassembly of the unisolated pump which caused a 30-40 gallon radioactive spill.
c) On May 26 and June 5, 1983, the operations staff failed to abide by the requirement of Station Directive 3.1.28 when incomplete operations surveillance procedures were signed off as being complete with no discrepancies resulting in the fa.ilure to perform the applicable surveil-lance requirements.
This is a Severity Level IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are'hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
