JP0/WHM August 26, 1983 s-Duke Power Company McGuire Nuclear Station Response to Violation 50-369/83-28-01 Violation 50-369/83-28-01, Severity Level IV:
Technical Specification 6.8.1.a requires that written procedures be established, implemented'and maintained covering surveillance testing and maintenance.
Contrary to the above, the licensee technicians and operations personnel failed to' abide by the. Technical. Specification requirements in that: a) on June 11, 1983, at 12:54 p.m., an electronic *echnician failed to follow Procedure IP-0-A-3010-05 during the performance of 'Jnit 2 SSPS functional test, resulting in actuation of.an improper relay which caused a reactor trip; b) on June 2, 1983, maintenance technicians failed to abide by'the requirements of Station Directive 3.1.19, when they failed to have the Unit 1 B Boric Acid Transfer pump isolated and logged prior to disassembly of the unisolated pump which caused a 30-40 gallon radioactive spill; c) on May 26 and June 5, 1983, the operations staff failed to' abide by the requirement of Station Directive 3.1.28 when incomplete operations surveillance procedures were signed off as being complete with no discrepancies resulting in the failure to perform the applicable surveillance requirements.
. Response to Part a:
Duke Power Company. agrees that during the McGuire Ne: lear Station Unit 2 SSPS functional test, test personnel inadvertently actuated an incorrect relay resulting in a reactor trip.- Personnel involved in this test included qualified technicians who were ~either. experienced or. it' experienced on SSPS equipment. As part of
~McGuire's training practices, technicians who are inexperienced on SSPS equipment are guided by experienced technicians-in order to gain experience on the equipment.
This violation occurred due to personnel error. The inexperienced technician failed to properly ensure'that.the correct relay was actuated, and in addition, the supervising technician did not adequately verify that the correct relay was selected. Personnel involved in the testing have been counseled and.all technicians, especially supervisory technicians, have been' reminded to be more attentive when accompanied by an inexperienced technician and to follow procedures. McGuire Nuclear Station is presently in' full compliance with the Technical' Specifications.
Response to Part b:
Duke Power [ Company, agrees that McGuire Nuclear' Station maintenance technicians
' failed to. isolate and' tag'the Unit 1B BAT pump prior to disassembly of the pump.
- This resulted in a 30-40 gallon radioactive spill. The cause of this occurrence was due'to personnel?not properly following a procedure which required an independent:
verification of safe working conditions, and placement of separate safety tags.
' Personnel.have been counseled on the importance of following procedures so that
. incidents such as this will'not occur. McGuire Nuclear Station is presently in full compliance with'the Station Directive.

-Response to Part c:
Duke Power Company agrees that operations personnel failed to follow procedures at. McGuire Nuclear Station. This violation involved missing surveillance require-ments, failure to sign off surveillances, and failure to verify that required surveillances were performed. This violation occurred due to personnel error.
Personnel involved were counseled, and crew meetings were held to stress the importance of completing the requirements of periodic tests. The importance l
of the supervisory function of reviewing and approving periodic test procedures has been reviewed with all Shift Supervisors. Additional investigations revealed that the surveillance procedure's format may have contributed to the personnel
. error; therefore, the format has been revised to reduce the possibility of future errors.
McGuire Nuclear Station is presently in full compliance.
