PVNGS UNIT 1 I.
DESCRIFIION OF DEFICIENCY During startup testing of the Unit 1 Diesel Generator System, Diesel Generatur "A" and "B" jacket water heaters M-DGA-M01 and M-DGB-M01 and lube oil warm-up heaters M-DGA-M02 and M-DGB-M02 were unable to maintain oil and water temperatures for engines M-DGA-H01 and M-DGB-H01 at the design requirements. Reference specification paragraph 4.4.2.9.2.2.9 requires a jacket water heater with thermostatic control to maintain the jacket water at 125'F minimum when engine is not in operation. Paragraph 4.4.2.8.5 requires an oil heater with suitable heating elements and thermostat to maintain required lub oil temperature. The function of the heaters is to maintain sufficiently warm conditions such that the diesel generator can start and be at a rated frequency and voltage within ten (10) seconds as required by project licensing documents and reference specification paragraph 4.4.1.5.
II.
ANALYSIS OF SAFETY IMPLICATIONS This condition is evaluated as reportable. A shop qualification program qualified this engine design by three hundred (300) starts with oil and water at nominal temperatures of 120' 10'F.
All starts attained the rated frequency and voltage in less than ten (10) seconds.
There is no assurance that the heating systems as described above will provide performance equal to the qualification tests. This condition is therefore potentially safety significant representing a significant deviation from performance specifications.
III.
CORRECTIVE ACTION 1)
Design Change Package 1SM-DG-029, 2SM-DG-029 and 3CM-DG-029 have been issued to install new heaters in Units 1, 2 and 3, re spective ly.
2)
Final Safety Analysis Report Sections 9.5.5 and 9.5.7 and Figure 9.5-12 will be revised. per Safety Analysis Report Change Notice 1095 to incorporate changes related to licensing documents.
In addition to reportability under 10CFR50.55(e), Bechtel PVNGS Project considers the deficiency to be Reportable under the requirements of 10CFR Part,21 by the supplier. Deficiency Evaluation Report 83-24 addresses the reporting requirements specified under 10CFR 21.21(b) (3) with the exception of sub part (vi) which' requires the number and location (customers and/or facilities) of other possible defective equipment. A copy of this report will be sent to Cooper Energy Services requesting their review for reporting under 10CFR Part 21 including number and location of all components supplied.
