BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - GENERIC DEFICIENCIES AT CONSOLIDATED CONTROLS CORPORATION - BLRD-50-438/82-68, BLRD-50-439/82 SECOND INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. V. Crlenjak on September 16, 1982 in accordance with 10 CFR 50.55(e) as NCR 1950. This was followed by our first interim report dated October 18, 1982. Enclosed is our second interim report. We expect to submit our next report on or about November 17, 1983 We consider 10 CFR Part 21 applicable to this deficiency.

SECOND INTERIM REPORT Description of Deficiency l
Consolidated Controls Corporation (CCC) of Bethel, Connecticut, has supplied TVA with the solid state control system (SSCS) as well as the basic control room switch modules (Type SN3) for the Bellefcnte Nuclear Plant. Since the beginning of shipment in May 1977 and continuing to the present, there have been at least 39 nonconformances written against the equipment supplied by CCC. At least six of these nonconformances have been determined to be reportable to the NRC.
Individually, most of these nonconformances do not represent a -
significant condition adverse to. quality; however, collectively, they do indicate a breakdown in the supplier's quality control program.
Bellefonte is the only TVA plant to receive equipment under a QA contract from CCC.
Interim Progress CCC has responded to the-QA/QC concertur of'tiris nonconfornnnce' report (NCl:)
in their letter to TVA dated December 15, 1982 (EEB.821217 017), and in

their responses to TVA audits in September 1981 and October 1982 and to the NRC audit in July 1982.
CCC has provided some failure analyses of failed equipment listed on other NCRs. Failure analyses on the remainder of failed equipment are due by August 1, 1983 Preliminary evaluation of their sub.aittals indicates that CCC has corrected their quality control problems and that numerors equipment failures arc random failures. TVA will fully evaluate quality coi. trol issues and discuss the resuli;s in a subsequent report.
