BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - INCORRECT INSTALLATION OF PRESSURE RELIEF AND SAFETY VALVES - BLRD-50-438/82-25, BLRD-50-439/82 FOURTH INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector Don Quick on March 5, 1982 in accordance with 10 CFR 50.55(e) as NCR BLN BLP 8210. This was followed by our interim reports dated April 2 and July 13, 1982 and January 7, 1983 Enclosed is our fourth interim report. We expect to submit our next report by July 18, 1984.

ENCLOSURE BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 INCORRECT INSTALLATION OF PRESSURE RELIEF AND SAFETY VALVES NCR BLN BLP 8210 BLRD-50-438/82-25, BLRD-50-439/82-22 10 CFR 50.55(e)

FOURTH INTERIM REPORT Description of Deficiency Condition I - The two relief valves on the component cooling water supply lines to the shell side of the letdown coolers discharge into a common waste disposal drain header. The waste disposal header is not sized to accommodate both relief valves. Also, there are several other smaller relief valves discharging into this same waste disposal header. This condition applies to units 1 and 2.

Condition II - Two relief valves on the component cooling water piping for the coolers to reacter crolant pump PTA1 discharge into a common line which is sized too small to accommodate both relief valves. This condition applies to unit 1 only.

These conditions violate ASME Code Section III, Division I, ND-3677.3d(2).

Interim Progress Condition I TVA has proceeded with the solution of exhausting the two 6-inch relief valves to containment atmosphere and adding two 3-inch relief valves set at a lower pressure which exhausts into the waste disposal header.

The design criteria diagram and unit 1 physical piping drawings have been issued. The relief valves have been procured. The issue of unit 1 support analysis has been rescheduled to July 1, 1983, due to other design changes brought about by NCR BLN BLP 8303 which affects this anlaysis problem.

This will delay completion of the piping system hanger design.

Condition II 1

The size of the common drain line from the two relief valves will be l

increased to accommodate flow from both relief valves as stated in the third interim report.

l The physical piping drawings, support analysis, and hanger design work has been delayed due to the addition of engineering changes brought about by NCR BLN BLP 8236.
