e Attachment to CP&L Letter of Response to NRC Report RII: GFM 50-400/83-12 Reported Violation:
10 CFR 50, Appendix B, Criterion V as implemented by CP&L PSAR section 1.8.5.5, CP&L Corporate QA Program, Section 6.2.5, and Site Construction Procedures CQC-2 and AP-IX-16, require that Deficiency and Disposition Reports be issued and controlled in accordance with procedures.
Contrary to the above, on March 3, 1983 two CP&L Deficiency and Disposition Reports, DDR 1306 and DDR 1301, were located by CP&L QA personnel only after an extensive search, and after they were located, it was found that due to the failure to adequately control the status and location of DDR 1301, the DDR had not received an evaluation during the maximum procedural allotted time, which is 14 days.
This is a Severity Level V Violation (Supplement II.E.).
Denial or Admission and Reason for the Violation:
The violation is correct as stated. Procedural guidelines were inadequate to ensure timely issuance of Deficiency and Disposition Reports (DDR's) and evalu-ations for reportability.
Corrective Steps Taken and Results Achieved:
DDR 1369 was written and issued on March 7, 1983 to identify the Nonconformance Control problem. DDR 1301 was located and distributed on March 3, 1983. The reportability evaluation was completed March 3, 1983 and determined not reportable under 10CFR21 and 10CFR50.55(e). DDR 1306 was located and issued on March 3, 1983.
The condition was evaluated and determined not to be reportable under 10CFR21 and 10CFR50.55(e) on March 4, 1983. DDR 1369 was resolved and closed on April 18, 1983.
Corrective Steps Taken to Avoid Further Noncompliance:
Procedure CQC-2 Nonconformance Control was revised on March 29, 1983 to provide specific guid<: lines for timely issue of DDR's.
QA and QC supervisors were given additional training on control of DDR's and other nonconformance reports. The training emphasized their responsibilities; the importance of accurately and clearly identifying nonconforming conditions; the timely issuance of DDR's; and the proper tracking and monitoring of reportability evaluations. Responsibility for tracking reportability evaluations has been escalated to a supervisory level individual.
