- 2. If the equipment in use during a survey fails or malfunctions, it must be repaired or replaced;
( otherwise, the survey team shall retreat to a safe area until the equipnent is replaced or repaired. -
- 3. When ccnducting field surveys from inside a vehicle, readings nust be taken with the probe outside the l vehicle.
- 4. Requirements of HPO/CO-4, Radiation Work Permits, are not applicable.

( 1.5 Appoint a group mesnber to stay in the Emergency Operations Facility and establish and maintain comuni-caticms from Field and Plant Survcy Groups in order to record curvey data for dose assessment purposes.
2.0 Plant Survey Group Leader shall:
2.1 Dispatch squads to survey areas in the plant and on-site.
2.2 Post areas as necessary to prevent the spread of contamination and accumulation of large doses of radiaticn to operating personnel. .
2.3 Relay the results of the surveys to the Radiation Protection Team Leader.
2.4 Provide plant survey group members with sufficient plant status of radiation and contamination hazards to allow them to maintain radiation and contamination exposure to within approved levels.
3.0 Plant Survey Group Members shall:
3.1 Obtain necessary equignent for conducting the surveys assigned by the Plant Survey Group Leader 4
in Step 2.1. Equipraent can be obtained from emergency kits in the Emergency Operations Facility or from ,
normal storage location of HP portable instranents.
3.2 Check that equignent is functional before leaving '
for surveys. Additionally, ensure you possess the proper personnel dosimetry and pocket dosimeter is zerced, prior

to leaving to conduct surveys.
3.3 Conduct surveys when directed to do so by the Plant ~
Survey Group Leader. Fill out appropriate sections of Appendix EP 205B-1. .
3.4 During transit to specified areas, observe the energized survey instruments for changes abcne background levels. Reference level is waist high fIczn the ground.
3.5 Use the 04 survey meter to scan close to ground surfaces *o detect deposits of contamination.
Obtain shielded and unshielded readirgs to determine Beta /Gama ratio.
3.6 Report data to the Plant Survey Group TEader via

4.0 Field Survey Group Leader shall:

4.2 Maintain comunications with survey teams.
4.3 Analyze and relay the collected survey data to the Radiation Protection Team Leader.
5.0 Field Survey Group Members shall:
5.1 Acquire equiprnent & maps as the situation requires from the Emergency Operations Facility. Ensure you carry proper personnel dosimetry for the surveys you are to perform and pocket dosimeters are zeroed prior to leaving to ccnduct surveys.
5.2 Inform senior ranking Security officer of vehicle license pl:te ntmber of field survey vehicles and personnel assigned to those vehicles. During transit to the specified areas, observe the energized survey instruments for changes above background levels. Reference level is waist high from h the ground.
i 5.3 Conduct surveys as assigned by the Field Survey Group Leader. (Fill out appropriate portions of Appendix EP 205B-2.)
Use Gi survey meters to scan close to ground surfaces to detect deposits of contamination. Obtain shielded and unshielded readings to determine Beta /Gama ratio. All air samples taken should be counted in the field. Refer to HPO/OO-60 thru 67 for methods to take the various field samples required.
5.4 Upcn conpletion of the surveys, return the vehicle to a predetermined area (preferably the Delta Service Building) for possible decontamination.
5.5 Survey the vehicle and decontaminate in accordance .
with HM/OO-8, Decontamination of Tools and Equignent, as necessary. If the vehicle was contaninated, inform the Field Survey. Group Leader.
5.6 Report data to the Field Survey Group Leader via " operator" radios.

1.2 Provide the survey teams with appropriate information as to plant status and expected radioactive " plume" movement which may affect the activities of the teatrs. Advise team members to be cognizant of "pitzne" and to move to edge of plume after data is collected. (Maximum advantage should be made of time, distance and shielding in accordance with AIARMA considerations) .
2.0 Radiaticn Protecticn Team Members ahll:
2.1 In case of failure of primary comunications radios, locate the nearest comercial (pay) telephone and pass information via this secondary means of cornunications to the Radiation Protection Team Leader in the Emergency Operations Facility.

FIRE AND DAMmr fEAM The Fire and Damage Team consists of the interim team members, the on-shift P.O.'s, APO's, and A.O.'s . The team is augmented by other personnel who are on-site or cal 1.ed in. The aucmenting forces are trained in fire fighting but not necessarily in the emergency plan. These augmenting forces cperate under the direction of the team members and leaders who have had emergency plan training. The augmenting forces are:

PERSO!stiEL SAFETY TEAM The personnel safety team is made up of on-shi ft P.O. 's , A.P.O. 's , and A.O. 's who are trained in the Emergency Plan Procedures. The team is augmented by other site personnel such as plant staff engineers, test engineers, health physics technicians, and others who are not necessarily trained in the Emergency Plan but will assist under the direction of the team leaders and a
members.
SEARCH AND RESCUE TFW4 The search and rescue team is organised by the Personnel Safety Team Leader at the direction of the Emergency Director. It will consist of personnel who are available which are knowledgeable of the plant layout and recognize conditions which could result from the existing emergency condit: n. The purpose of this team is to search plant areas quickly to locate and rescue victims. Personnel who know plant layout and are available make up,this team are from the following groups:

PUREOSE:
'Ihis appendix provides the method of contacting key staff personnel' who will, in-turn, notify sufficient support personnel to meet the one-hour staffing requirement. It is used if an Emergency Plan Alert, Site Emergency, or General Bnergency is declared.
- 1. 'Ibe follcwing step shall be performed, in sequence, by the Shift Clerk or other assigned personnel.
- 2. Inform each person ccntacted of the event classification and that 60 minute staffing is required.
- 3. Fall pager numbes. if the phone is busy, if there is no answer, or if you know that the individual is "on-the--pager". Individuals
( may, of course, supply temporary numbers which should be
<-allal first. .
- 4. Ccntact two staff senior engineers. W. T. Ullrich is contacted
- w- by the Convnunicator and counts as one contact. Do not .
