Enclosed is our first interim report. We expect to submit our next report by July 13, 1983 We consider 10 CFR Part 21 applicable to this deficiency.

FIRST INTERIM REPORT l

j Description of Deficiency The auxiliary feedwater (AFW) pump room personnel access doors exhibit weld quality not in conformance with American Welding Society (AWS) D1.1 acceptance criteria. This is contrary to the requirements stipulated in TVA's Procurement Specification 3294, Section 23 The documented weld deficiencies include undersized, overlapped, and undercut welds, craters and slag. Undersize welds are present in a high proportion of the assemblies. The doors were fabricated by Johnson Machine Works, Chariton, Iowa.

Nonreportable nonconformance report (NCR) 1463 and reportable NCRs 1630 and 1929 were previously initiated to document similar deficiencies with Johnson Machine Works supplied items. This NCR and NCR 1929 were issued as a result of significant conditions adverse to quality documented on the earlier NCRs and a subsequent investigation to determine if a generic condition adverse to quality with respect to Johnson Machine Works products exists.

Interim Progress TVA is analyzing the subject welds to determine if they will perform their intended secondary safety function in their present condition. TVA will provide an cdditional report upon completion and review of this analysis.
