  • ttachment to CP&L Letter of Response to NRC Report RII:

RWW 50-400/82-26 Reported Violation:

10 CFR 50, Appendix B, Criterion XV, the accepted QA program (PSAR Section 15),

and construction procedure TP-17, Construction Inspection Nonconformance Control, require that nonconforming conditions be reviewed and accepted, rejected, repaired or reworked in accordance with documented procedures.

Contrary to the above, a nonconforming condition was not being resolved as required by construction procedure TP-17.

Reinforcing steel inspection records for the Unit 1 emergency service water intake structrue slab concrete placement lISSL262010 poured on August 2, 1982, revealed that seven No. 6 rebars were clearly identified by the CP&L Construction Inspector as being discrepant because they were cut below their design projection to make room for an electrical blockout; however, this nonconforming condition did not receive engineering review and acceptance prior to placement of the slab.

This is a Severity Level IV Violation (Supplement II).

Denial or Admission and Reason for the Violation:

The violation is correct as stated.

The violation occurred due to an oversight of the cognizant Construction Inspector.

Corrective Steps Taken and Result Achieved:

The nonconforming condition was documented on Discrepancy Report DR-C-1519-R1, 8/6/82, and resolved on the same day by issue of Permanent Waiver PW-C-3470-R1 to accept the condition as-is.

Previous placements inspected by the Construction Inspector in question were reviewed for similar occurrences. No other cases of inspector oversight were found.

Corrective Action Taken To Avoid Further Noncompliance:

A refresher training session was conducted for rebar inspectors, including the inspector involved in the reported violation.

Emphasis was placed on obtaining corrective action for identified discrepancies prior to inspector acceptance and sign off of work items.

Date When Full Compliance Will Be Achieved:

Full compliance was achieved on August 6, 1982.
