SUMMARY
- OCTOEER 1978 The unit remained shut down through the month for refueling (and maintenan Full core invessel sipping was completed, revealing eleven
- 11) leaking fuel assemblies. Core loading was completed on October 19, 1978.
On October 3,1978, the new liquid / solids radioactive waste system was placed in service.
Initial startup activities and problem solving continues.
While performing routine inservice inspection within the reactor vessel, a circumferential through wall crack was discovered in Core Spray System II sparger. A repair program is being developed as of the end of the report period.
Five (5) Reportable Occurrences were identified during the report period:
RO #78-21 was identified on October 17, 1978, when a crack was i
discovered in Core Spray System II sparger.
R0 #78-22 occurred ~on October 17, 1978, when standby gas treatment I
system valve V-28-30 failed to open during routine i
surveillance testing.
l R0 #78-23 oc' curred on October 18, 1978, when two (2) intermediate range monitors were inoperable in the refuel mode.
R0 #78-24 was identified on October 18, 1978, during inspection.of the off gas system. The off gas particulate filter had burned during an off gas detonation and two (2) Valves were damaged.
R0 #78-25 was identified on October 31, 1978.
A leak in the bottom of the condensate storage tank was discovered which caused release of condensate into the ground under the tank.
