II. Scope  !

This report is provided in accordance with Reporting Requirements specified in. I NRC Bulletin 88 10 as it pertains to actions taken by Southern California i Edison (SCE) for the San Onofre Nuclear Generating Station (SONGS) during the  :

Unit 3 Cycle 5 Outage, i This report complies with the submittal commitments stated in the original ~ SCE response for addressing Item 1.c and Item 2 of the Reporting Requirements specified in NRC Bulletin 88-10.  !

This re) ort also contains the testing results for an additional MCCB.

Althoug1 previously identified for corrective action, MCCB record ID 318 has been determined in accordance with NRC Bulletin 88-10, Supplement 1, Position 3 to be not subject to these reporting requirements because it was received  !

and installed prior to August 1, 1983. It was tested, however, and is y addressed in Section VI.  :

This report completes the actions required by the NRC Bulletin for the Unit 3 Cycle 5 Outage as identified in Non Conformance Report (NCR) 3-2364.

III. Review of Previous Report Information This report provides information as a result of testing or replacing MCCBs during the Unit 3 Cycle 5 outage as committed to in the original SCE response.

A. Breakers Replaced and not Tested Of the six MCCBs listed, three were initially scheduled to be replaced rather than tested. These three MCCBs were replaced during the Unit 3 Cycle 5 Outage with new MCCBs arocured in accordance with item 7 of the NRC Bulletin.

Specifically, t1ese MCCBs were supplied under 10CFR50 Appendix B from Square D l

in Peru, Indiana. Provided below is a tabulation of the Record ID, Equipment 10, the maintenance work order under which each MCCB was replaced and the purchase order for which the replacement MCCB was obtained.

Contrary to this requirement, MCCB Record ID 280 was disassembled and scrapped by M090012788 before the n -

i NRC Bulletin 68-10 -3 l Outage Report Unit 3, Cycle 5 above actions could be implemented. As stated in our NRC Eulletin 88-10 follow-up Report dated September 6,1989, MCCB Record 10 280 was purchased in 1985 from Orange Coast Electrical Company and could not be directly traced to a circuit breaker manufacturer. This supplier is not on the NRC list of i suppliers of refurbished MCCB's. A previous limited inspection of this MCCB '

in its' installed configuration showed no indication of refurbishment. The MCCB appeared new with no signs of tampering. This breaker is a single pole design with rivetted casing and, because of this design, would have been very i difficult to refurbish without visible indications. It is doubtful, I therefore, that the MCCB would have been declared suspect had the planned l visual inspection and evaluation been performed. I B. Breakers Tested The remaining three MCCBs were tested in accordance with Verification Test Procedure VTP-E067. This )rocedure complies with Attachment 1 of NRC Bulletin 88-10 for implementing eac1 of the seven MCCB testing requirements. Of the three MCCBs, two were tested successfull;. and returned to service during the Unit 3 Cycle 5 Outage. The results of the testing for these MCCBs are discussed in Section V below.

The MCCB which failed the test was re) laced with an MCCB which complies with ,

the criteria specified in Item 5 of tie NRC Bulletin for acceptable  !

replacements. The replacement MCCB (10 269-1 as identified in Appendix A of SCE's September 6, 1989 Report) was obtained from quarantined (nontraceable) stock but was successfully tested in accordance with SCE's Verification Test Procedure VIP-E067. (Reference SCE CWO 89122220).

V. Test Results for Installed MCCBs This section provides the test results for three MCCBs tested during the Unit 3 Cycle 5 Outage which are subject to the reporting requirements of NRC Bulletin 88 10. Tabulated below for each MCCB are the Record ID, manufacturer and model number. The procurement chain for each MCCB listed below was provided in Ap)endix C of SCE's followup resort for Bulletin 88-10 which was submitted to tie NRC by letter dated Septem)er 6, 1989. A maintenance work order is identified for each MCCB installation and a construction work order is identified for each MCCB tested.

B of SCE's March 30, 1989 report) was also tested and replaced during tie Unit 3 Cycle 5 Outage. Though initially addressed for action, this MCCB was later determined ia sccordance with NRC Bulletin 8810, Supplement 1, Position 3 to be not sube to these reporting requirements because it was received and installed pi ..r to August 1,1983. This MCCB is identified as follows:

Because this MCCB was already evaluated under the requirements of Bulletin 88-10, we decided to include MCCB 318 in this report.

MCCB 318 was tested in accordance with Verification Test Procedure VTP-E067.

The MCCB failed Step 3.7,135% Rated Tri) Test. In accordance with this test, the MCCB should have tripped within one lour. The MCCB did not trip at either the high or low setting.

Because of this test failure, MCCB 318 was re) laced with a MCCB which complies i with the criteria specified in Item 7 of the au116 tin. Since the tested MCCB l model is now obsolete, a substitute MCCB was obtained under Appendix B from

'The MCCB failed Step 6.5 of VTP-E067, 100% Rated Hold-In Test. The test required a hold in time of two hours. The MCCB tripped in 72 mir.utes.

NRC Bulletin 88-10 5- i Outage Report Unit 3, Cycle 5 Square D in Peru, Indiana (Reference SCE PO 6A040009). The replacement MCCB '

was evaluated under Substitution Equivalency Evaluation (SEE) 90 0023. The construction work order which tested MCCB 318 and the maintenance work order which installed the new MCCB are identified below.
