This refers to the inspection conducted by Messrs. T. Taylor, K. Walton,

P. Prescott, and R. Roton of this office from October l1 through

November 5, 1993. This inspection included a review of authorized activities-

for your Quad Cities Nuclear Power Station, Units 1 and 2.
At the conclusion
of the inspection, the findings were discussed with those members of your'
staff identified in the enclosed report.

Areas examined during the inspection are identified in the' report ~. Within
,
these areas, the inspection consisted _of a selective examination of procedures!
and representative records, interviews with personnel, and observation .of -
'
activities in progress.
During this inspection, certain of your activities appearedLto be in violation:
of NRC requirements, as specified in the enclosed Notice' of-Violation

(Notice). This violation is of concern to the NRC.
Corrective _ actions taken
y
to control performance of unauthorized plant activities were inadequate to-
prevent recurrence.
The unauthorized activity was a test of computer-room
ventilation equipment that unexpectedly-resulted in a~ halon system actuation.
There were two previous instances-of halon system actuations under similar
circumstances with no actions taken to prevent recurrence.
