Dear Mr. Pleniewicz:

This refers to the inspection conducted by Messrs. T. Taylor, K. Walton, ,

P. Prescott, and R. Roton of this office from October l1 through .

November 5, 1993. This inspection included a review of authorized activities-

for your Quad Cities Nuclear Power Station, Units 1 and 2. At the conclusion

of the inspection, the findings were discussed with those members of your' '

staff identified in the enclosed report.

Areas examined during the inspection are identified in the' report ~. Within ,

these areas, the inspection consisted _of a selective examination of procedures!

and representative records, interviews with personnel, and observation .of -  :

During this inspection, certain of your activities appearedLto be in violation:

of NRC requirements, as specified in the enclosed Notice' of-Violation j

(Notice). This violation is of concern to the NRC. Corrective _ actions taken y

to control performance of unauthorized plant activities were inadequate to-

prevent recurrence. The unauthorized activity was a test of computer-room

ventilation equipment that unexpectedly-resulted in a~ halon system actuation.

There were two previous instances-of halon system actuations under similar

circumstances with no actions taken to prevent recurrence.

You are required to respond to this letter and should follow the' instructions!

specified in the enclosed Notice when preparing your response. In your

response, you should document the' specific actions taken and any additional q

actions you plan to prevent recurrence- After reviewing your' response to this: 'l

Notice, including your proposed. corrective actions and the results of future l

inspections, the NRC will determine whether further NRC~ enforcement action is. j

necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of ,

this letter, the enclosures, and your response' to this letter will be placed I
