DUKE POWER COMPANY McGUIRE NUCLEAR STATION RESPONSE TO IE INSPECTION REPORT 50-369/82-17 and 50-370/82-12 Violation 50-369/82-17-03, Severity Level V:
10 CFR 50, Appendix B, criterion V and the accepted QA Program, Section 17.2.5, require that activities affecting quality shall be prescribed by documented procedures and that these activities shall be accomplished in accordance with these procedures. The accepted QA Program, Table 17.0-1, states that the licensee conforms to Regulatory Guide 1.39 (Revision 1) which endorses ANSI N45.2.3-1973.
Station Directive 3.11.0, Housekeeping and Cleanliness, Revision 7 dated April 1981, implements the cleanliness and housekeeping program and states the following for zone designations of Level IV: Smoking or use of tobacco products is not permitted, and housekeeping areas will be marked by prominently displayed signs.
Contrary to the above, activities affecting quality were not accomplished in accordance with the approved procedure in that the following items were identified during the inspector's tour of the lower annulus on May 14, 1982; 1.
The entry to the lower annulus was not marked by a prominently displayed sign; and 2.
Numerous cigarette butts and crumpled cigarette packs were found in the annulus area.
Response
1.
The station agrees the violation occurred as stated.
2.
Due to the numerous signs posted at the lower annulus entrance, the absence of a housekeeping / cleanliness sign was not promptly noted and corrected.
Continued isolated cases of level IV cleanliness violations (smoking) by persons working at the station was the cause of this violation.

A level IV cleanliness sign was posted at the entrance to the lower annulus. A general plant survey was made to determine if other signs were defaced, covered, or missing. No other discrepancies were noted.
The annulos under went an extensive re-cleaning during the current outage.
The entrance area to lower annulus was the only location noted where level IV cleanliness violations were found.

The monthly safety inspection of the plant by station personnel will include checking the location and condition of cleanliness level signs.
A company " task force" is currently developing a program for signs and labeling that will include permanent and standardized cleanliness /
housekeeping signs.
Page 2 All station personnel have been informed verbally and in writing of cleanliness requirements. This communication also established a strict disciplinary policy which will be applied to any individual found vio'.ating cleanliness requirements.
A dedicated management effort began on June 29, 1982 to monitor areas where no eating, smoking, drinking, or tobacco chewing are allowed. The purpose of this effort is to identify any personnel violating cleanliness requirements with the intent of taking appropriate disciplinary action. The surveillance program will be continued until the problem has been corrected.

The station is presently in full compliance with Station Directives in this area.
