APPENDIX A Western Piping & Engineering Company Inc.
Docket No. 99900302/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on May 17-21, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantita-tive or qualitative acceptance criteria for cetermining that important activ-ities have been satisfactorily accomplished."
A nonconformance with these requirements is as follows:
Paragraph 12.4.3 of Section 12.0, " Control of Measuring and Testing Equipment,"
dated August 5, 1981, of the Quality Assurance Manual states, "The QC Manager is responsible for maintenance of the Inspection Usage Log which is a record of in-tolerance condition and frequency of use.
Recording of the checks performed by the QC inspector prior to each product acceptance inspection eliminates the need for an extensive recall system."
In addition, para-graph 12.4.5 states in part, "By utilizing the Inspection Usage Log, the QA manager determines which items were inspected between calibrations."
Contrary to the above, three reasuring instruments had not been entered in the Inspection Usage Log prior to use by the QC inspector for final acceptance inspection of a completed product.
