Dear Mr. Keppler:

This letter serves as the final report as required by 10CFR50.55(e) concerning potential defects in certain valve actuators supplied to the Perry Nuclear Power Plant. This subject was firs t reported to Mr. James Konklin of your office on July 8, 1982, by Mr. R. L. Vondrasek of The Cleveland Electric Illuminating Company.

This report contalns a description of the deficiency, an analysis of the safe ty implications, and the corrective action.

Description of Deficiency On June 11, 1982, the G. H. Bettis Company and the Henry Pratt Company each notified the Nuclear Regulatory Commission in accordance with 10CFB21 that a defect had been Ldentified concerntrg Bettis actuators installed on Henry Pratt valves. This notification was transmitted to CEI by Henry Pratt's letter dated June 18, 1982.

G. H. Bettla Co., Model T3 series actuator yokes with 3/8" deep keyway and 3" diameter shaf t bore appear to have insuf ficient remaining wall thickness for full-range torques. When the actuator operates, the yoke which is coupled to the valve shaf t by a key may be overs tressed, resulting in yoke failure due to cracking at the keyway and rendering the valve inoperable.

Eight valves supplied to Ferry Plant equipped with Bettis Model T312-SR2 actuators (containing the subject yokes) are Henry Pratt Co. 24" butterfly valves (B/M Item RNP-2); Tag #1M14-F055A&B; 1M14-F060A&B; 2M14-F055A&B; 2M14-F060A&B. Unit 1 valves have been issued from the warehouse, but not installed; Unit 2 valves have not been issued.

During operation the valves have to close watertight. The subject valves at Perry would have a high probability of failure in the manner described.

Corrective Action -

Site Nonconformance Report OPQC 258 has been initiated to track the deficiency a t Per ry. We have contacted G.- H. Bettis Company regaruing replacement of parts to correct the deficiency. G. H. Bettis has submitted a detalled procedure for correction of the deficiency. Currently the procedure is being evaluated by us to determine whether site personnel or a bettis representative will perform the work.

The estimated lead time for the replacement parts is three months. Upon recelpt of the parts, changeout is estimated to take one week.

Based on this information all work should be completed by November 29, 1982 Changeout will be closely monitored' by CEI's Nuclear Quality Assurance Depart-ment. Upon replacement of the defective parts, the valves will perform their intended safety function. .
