APPENDIX A NOTICE OF VIOLATION j

Based on the results of the NRC inspection conducted on October 30 -
l November 2,1978, it appears that certain of waur activities were not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in our correspondence to you dated December 31, 1974.
A.
Criterion V of Appendix B to 10 CFR 50, as implemented by Topical
()
Report, Duke 1 A, Paragraph 17.1.5, requires in part that, " Activities affecting quality shaLL be prescribed by documented instructions, procedures... and shall be accomplished in accordance with these instructions, procedures,...." Speci fication Number P815-1109.00-00-0001, concrete for Category 1, Structures, and referenced ACI 304-73 require concrete aggregates be stored in a manner that prevents overlapping of different sizes.
Contrary to the above procedure, on November 1,1978, while inspecting concrete aggregate storage facilities, the inspector found fine and course aggregate stockpiles exceeding their storage capacity and spilling over on one another.
This is a deficiency.
B.
Criterion V of Appendix B to 10 CFR 50, as implemented by Topical Report, Duke 1A, Paragraph 17.1.5, requires in part that, " Activities affecting

quality shall be accomplished in accordance with these instructions, procedures,...." Procedure M-26, Laboratory Soil Testing, and referenced l
ASTM 0422 require that sieves used for grain-size analysis conform to the requirements of ASTM E11.
Contrary to the above, discussions with responsible engineers indicated l
sieves used for grain size analysis are not checked for conformance to 1
ASTM E11.
This is an infraction.

Criterion XVII of Appendix B to 10 CFR 50, as implemented by the PSAR, paragraph 17.0 (Topical Report paragraph 17.1.17.4), requires in part that record retention shall be consistent with applicable regulatory l
requirements.
