APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation Docket No. 50-220 Based on the results of an NRC inspection conduc' ed on September 5-8, t
1978, it appears that one of your activities was not conducted in full compliance with the conditions of your NRC license as indicated below.
This item is a deficiency.
Technical Specification 6.8.1 states in part: " Written procedures and administrative policies shall be established, implemented and maintained..."

Administrative Procedure APN-15, Procedure for Control and Calibration of Equipment used in Tests and Inspections, specifies in section 4.2.C, Use Records, "Each portable device subject to use at intervals between periodic calibrations shall have a protected log sheet accompanying it which shall be used to log all devices calibrated or tested by said instrument during the period between calibrations."
Contrary to the above, on September 7, 1978, it was determined that the requirements of APN-15 were not adhered to in the area of "Use Records" as follows:
A calibration performed with a test instrument was not logged on its use record log.
A calibration performed with a test instrument was logged on the wrong test instruments use record log.

A deadweight tester, out for calibration, used as a standard in the performance of instrument calibrations had no use record log on file.
A deadweight tester currently in use had no entries in its use record log.
