SUBJECT: NOTICE OF DEVIATION

INSPECTION REPORT N05. 50-361/93-36 AND 50-362/93-36

This letter refers to the inspection conducted by Mr. C. Myers of this office,

during the period of December 6 through 10, 1993, of activities authorized by

NRC License Nos. NPF-10 and NPF-15, and to the discussion of our findings held

with members of your staff on December 10, 1993.

Specific areas examined during this inspection are described in the enclosed i

inspection report. Within these areas, the inspection consisted of selective 1

examinations of procedures and representative records, and interviews with

personnel.

Based on the results of this inspection, certain of your activities appeared  !

'

to deviate from your commitments, as specified in the enclosed Notice of l

Deviation (Notice). The deviation concerns your calculational method for MOV i

capability under degraded voltage conditions. In addition, a violation  !

concerning MOVs reported to be inoperable is not being cited due to your self- ,

identification and adequate corrective actions. The violation is not being j

cited because it meets the enforcement criteria of 10 CFR Part 2, Appendix C,  !

VII.B. 1

Paragraph 2.7 of this report discusses recent maintenance related deficiencies  !

observed with motor operated valve (MOV) 3HV-6371. The valve internals were

found to be damaged apparently due to prior misassembly. Your staff was

conducting a maintenance division investigation of the cause of the observed

problems. Repeated indications of significant abnormal MOV operation were

displayed during diagnostic testing of this valve dating back to March 1992.

We are concerned that the maintenance related deficiencies which you have I

identified in 3HV-6371, and the apparent delay in addressing abnormal . testing

results with that valve, appear to be another example of a weakness in your )

program to assure that your MOVs are operable and maintained operable. '

Another example of this apparent weakness is discussed in Paragraph 2.1.
