SUBJECT:

NRC INSPECTION REPORT NO. 50-458/93-26 (NOTICE OF VIOLATION)

This refers to the inspection conducted by Mr. W. F. Smith, and the inspectors

identified in the attached report, of this office on August 22 through

September 25, 1993. The inspection included a review of activities authorized

for your River Bend Station facility. At the conclusion of the inspection,

the findings were discussed with you and those members of your staff

identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examination of procedures

and representative records, inspections of installed equipment and system

lineups, interviews with personnel, and observations of activities in

progress.

The purpose of the inspection was to determine whether activities

authorized by the license were conducted safely and in accordance with NRC

requirements.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The first violation is of concern because the operators

were not provided with procedural guidance to respond to a process radiation

monitor alarm that indicated that an unexpected radioactive discharge from the

main cooling tower blowdown line was occurring.

The second violation is of

particular concern because the operators failed to promptly implement measures

to identify and correct the condition which resulted in an unexpected high

radiation alarm received during a combined liquid radioactive waste and main

cooling tower blowdown discharge to the environment.

Consequently, over

500,000 gallons of potentially radioactively contaminated water was discharged

to the Mississippi River.

Two unresolved items were identified during this inspection.

The first

.

unresolved item identified in Section 2.3 will remain open pending the staff's

review of the implementation of the vendor technical information program and

the motor-operated valve operability concerns.

The second unresolved item,

identified in Section 5.2, will remain open pending the staff's review of the

remote shutdown panel operability deficiencies.
