To discuss the failure to take prompt corrective actions for hardened grease in 4 kV and 6.9 kV circuit breakers following a 10 CFR Part 21 notification and an October 1992 breaker failure; identify root causes and take timely corrective actions to prevent repetitive failures of secondary containment isolation (VR) dampers to close and; identify testing deficiencies and take appropriate corrective actions to repetitive problems with safety relief valve performance.
