SUBJECT:

ROUTINE RESIDENT INSPECTION REPORT AND NOTICE OF VIOLATION

This refers to the routine safety inspection conducted by J. D. Smith,

M. J. Miller, V. P. Lougheed, and S. G. DuPont of this office from

November 24, 1993, through January 6, 1994. This inspection included a

review of activities authorized for your Zion Nuclear- Generating Station. At

the conclusion of the inspection, the findings were discussed with those

members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report.

Within these areas, the inspection consisted of selective examinations of

procedures and representative records, interviews with personnel, and

observation of activities in progress.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation is of concern because it illust. rates'a

failure to verify the satisfactory performance of safety equipment.

Appropriate criteria.were not rigorously developed and applied, creating'a

potential for unsatisfactory equipment performance to be unrecognized and

un;orrected.

In addition, this . demonstrates a need to ensure that the plant

design basis information is documented and disseminated to all plant employees

who a ed it.

In addition,.during this inspection period, certain other of your activities-

appeared to be in violation of NRC requirements. Specifically,-a technical

specification surveillance was missed and offsite review of a safety

evaluation was not performed. However, as described in the enclosed

inspection report (section 8), you identified these violations. Therefore,

the violations will not be subject to enforcement action because your efforts

in ' identifying and correcting the violations met the criteria specified in-

Section VII.B.2 of the " General Statement of Policy and Procedure for NRC

Enforcement Actions" (Enforcement Policy, 10 CFR Part 2, Appendix C).

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence.

After reviewing your response to this Notice, including your proposed

corrective actions and the results of future inspections, the NRC will

determine whether further enforcement action is necessary to ensure compliance

with regulatory requirements.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.
