Joseph M. Farley Nuclear Plant Unit 1 Narrative Summary of Operations -

December 1993 There were no unit shutdowns or major power reductions during the month of December.

The following major safety related maintenance was perfonned during the month:

1.

Performed miscellaneous corrective and preventive maintenance on the diesel generators.

2.

Modified the 1B charging pump gear box to prevent an oil mist problem.

3.

Rebuilt the IC service water pump (A Train) Breaker due to tripping.

4.

Performed maintenance on two main feed regulating valves due to flow oscillations.

Joseph M. Farley Nuclear Plant Unit 2 Narrative Summary of Operations December 1993 The cycle 9-10 refueling outage continued through the month of December. Power operation began at 1232 on 12-02-93.

At 2233 on 12-02-93, with the unit in mode 1 operating at approximately.5 percent reactor power, the reactor tripped due to a low-low water level in the 2C steam generator (S/G) following a turbine trip and feedwater isolation. The unit returned to power operation at 1944 on 12-03-93.

The generator was taken offline for a generator overspeed trip test. The reactor remained critical during the test. The unit returned to power operation at 2359 on 12-03-93.

At 1912 on 12-21-93, the unit was shut down for a maintenance outage due to excessive vibration in the main turbine. It was discovered that the inner diaphragm gasket on the flange on the inlet to the low pressure turbine #2 had failed. The failed gasket allowed an incorrect steam flowpath, which induced high vibrations in the turbine.

j The gasket was repaired and a visual inspection of the turbine was completed satisfactorily.

The unit returned to power operation at 1831 on 12-29-93.

The following major safety related maintenance was performed during the month:

1.

Performed miscellaneous corrective and preventive maintenance on the diesel generators.

2.

Replaced spent fuel pool radiation monitor RE25B due to it spiking high.

3.

Modified the 2A and 2B charging pump gear boxes to prevent an oil mist -

problem.

4.

Replaced the letdown orifice outlet relief valve Q2E21V0255 due to seat leakage.

t 5.

Replaced controller card on main steam atmospheric relief valve 3371 A.
