I. 1 INTRODUCTION-

.The! Inservice Inspection was performed during the Unit 2 third refueling

, outage which.beganuon March-16, 1990. .The scope ofLinspections included

- ASME Section XI requiredienaminations, 'and N.R.C. required augmented examinations .' - All. examinations performed were.in~accordance with the LaSalle County Station Unit 2. Technical-Specifications and ASME'Section XI' 1980 Edition including the 19801 Winter. Addenda.'

-t General Electric Co.'(GE) was contracted;to performLthe Non-Destructive Evaluation's (NDE):of the reactor vessel velds, piping welds, and pressure =

boundary attachment' welds. . Magnetic particle (MT),111guld dye penetrant, .i (PT), and manual'or' automatic, ultrasonic (UT) techniques.were usedEduring 1 these inspections. :Personne1'from Comm~mwealth Edison?s System Material

' Analysis Dept.:(SKAD) and LaSalle Statig s Technical Staff provided any-necessary resolution ~of technical' problems,-and reviewed all I.S.I.-

. personnel qualifications, Land reviewed all visual, surface and volumetric '

examination-results. The Level-II and Level III inspection personnel. j performing UT were qualified for detection and discrimination of

Intergranular Stress Corrosion; Cracking (IGSCC) byithe Electric Power Research Institute (EPRI) at the-EPRI NDE center after September 10, '!

1985.' Level I pars' N1 not qualified. at EPRI did. notJperform UT scanning duties, although'th je uced as helpers in the set-up of the automatic UT. They were trained'in set-up activities by EPRI: qualified G.E.

personnel.

4 Visual examinations'of' reactor' vessel' internals, class 1 bolting,.

component supports, and piping pressure tests were: performed by CECoi.G.E. <

and Sargent & Lundy : employees. These,3nspectors were currently certified as ASME Section XI Visual Examiners through the CECO qualification: . '

program. An underwater camera was used in the= reactor vessel for the visual inspection of reactor vessel internals...;The camera was proven capable of resolving a 1 mil stainless steel' wire while underwater and in the same lightiny conditiens found during thel actual" inspection.1 The services of en Authorized Nuclear _ Inservice Inspector'

( ANII)/Authr rized Nuclear Inspectorq ( ANI) were provided by Hartfori Steam -

Boller Inspection & Insurance:Co., 300 S. Riverside Pl., Suite 700.N.,

Chicago, IL 60606. The ANII/ANI, John Tetreault,-National Board 88796, State of IL 11416, reviewed all procedures, personnel qualifications,  ;

instrument and material certifications, all; examination results, and repairs / replacements.

be listed on the form NIS-1. These components include,the Reactor Vessel; i the RHR Heat Exchangers, the Class 1 &:2 Piping Systems and the Class.1;&

i 2 Component Supports inspected during the^ refueling outage and will be

[ listed on the form NIS-1,-Item 58. A more-detailed list of components L inspected will appear in the abstracts listed as Items 10, 11, and 12 on L

the form NIS-1. Since descriptions' including size, capacity, material, location and drawings to aid in identification h've a been previously submitted in our N-5 data reports and also our I.S.I. program, they will not be included in the summary report but are on file at LaSalle. This also applies to our preservice and previous inservice inspection summary reports.

SUMMARY

OF RESULTS AND CORRECTIVE ACTIONS

The ASME Section XI inservice inspection results obtained this outage  !

were compared with the preservice and previous inservice inspection , t results. The following is a summary of the results and the corrective ,l actions taken this outage.  ;

1. 415JetPumpShnsingLineBracket -.  !

During the Unit 2 third refueling outage, ISI of the Rs. vessel

, internals revealed cracks in the attachment welds bette.en the sens'ing  ?

line and the lowest sensing line bracket for the #15 jet pump. .An analysis by General ~ Electric determined'that this condition left as is for the next cycle may result'in coditional damage up_to a break-  ;

in the-sensing line-itself. General Electric recommended that a'  ;

temporary clamp be installed during this refueling, outage.- CECO's  !

Nuclear. Engineering Department reviewed the proposed clamp design and - '

found it acceptable for' installation on the 515 jet pump.' The clamp was then installee as a temporary system change. CECO's Nuclear -;

Engineering Department will-investigate ant provide recommendations- l for a permanent modification to the sensing line support configuration.

2. Steam Dryer During the Unit 2 third refueling outage, ISI of the steam dryer ,

revealed cracks in the steam dryer support ring. The steam dryer support ring (3.5" x 9.5") was fabricated from 304SS tanealed flat plate which is cut and rolled to shape. The dryer skirt,' drain channels and banks are then welded to the ring. General Electric has reviewed the video inspection tapes of'the indications found on the 4 steam dryer eupport ring and has concluded the cracking is acceptable and do not need to be removed or repaired before startup. CECO's q Nuclear Engineering Department concurs with General Electric and recommends the following actionst

a. During L2R04, perform an ultrasonic inspection of those indications found in the Unit 2 steam dryer support ring to l determine the depth of-the cracking.

, b. Crack depth should be sized again-during L2R05 to determine tLs .
