Yelverton, Vice President Operations, Arkansas Nuclear One Route 3, Box 137G Russellville, Arkansas 72801 , SUBJECT: NRC INSPECTION REPORT 50-313/93-26; 50-368/93-26 (NOTICE OF VIOLATION) This refers to the Fuel Integrity and Reactor Subcriticality (FIRS) and inservice inspection conducted by Mr. L. E. Ellershaw, and the-inspectors identified in the enclosed report, of this office on September 7-24, with in- office inspection of records until October 8, 1993. The inspection included a review of activities authorized for your Arkansas Nuclear One, Units 1 and 2, facility. At the conclusion of the inservice inspection, an exit meeting was held on September 17, 1993, and the findings were discussed with you and those members of your staff identified in the enclosed report. Additionally, at the conclusion of the FIRS inspection, a preliminary exit meeting was held on September 24, 1993, and the findings were discussed with you and those members of your staff identified in the enclosed report. Further in-office review pertaining to the FIRS inspection was performed during September 27 through October 8,1993, and the results of those reviews were telephonically reported to your representatives Mr. R. King, Acting Director of Licensing, and Mr. S. Bennett, Acting Licensing Supervisor, on September 30 and October 8, 1993. Areas examined during the inspection are identified in the report. These areas included in-core fuel loading and fuel storage configurations, core component performance, outage work controls and critical path scheduling, potential for fuel-related problems identified at other facilities, fuel handling procedures and practices, disposition of degraded core components, and inservice inspection. Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observation of activities in progress. Based on the results of this inspection, certain licensed activities appeared to be in violation of NRC requirements, as specified in the enclosed Notice of Violation (Notice). The violation is of concern because it indicated that work controls related to controlling foreign material in restricted areas were not effective and it is similar to a previous non-cited violation. . In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its enclosures will be placed in the NRC Public Document Room. The responses directed by this letter and the enclosed Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, Pub.
