SUBJECT:
NRC INSPECTION REPORT 50-285/93-23 (NOTICE OF VIOLATION)
This refers to the inspection conducted by Mr. L. T. Ricketson of this office
on October 12-15, 1993. The inspection included a review of activities
authorized for your Fort Calhoun Station facility. At the conclusion of the
inspection, the findings were discussed with those members of your staff
identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of

activities in progress.
The purpose of the inspection was to determine-
whether activities authorized by the license were conducted safely and in
accordance with NRC requirements.
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
The violation is of concern because _it involved several
separate occurrences of a failure to adhere to radiation work permit
requirements, and licensee corrective actions after each occurrence were not
sufficient to prevent subsequent occurrences.

The inspection also identified that an entrance to a high radiation area with
radiation dose rates greater than 1000 millirems per hour was not locked to
prevent unauthorized entrance. This was identified as a violation of
Technical Specification 5.11.2.
However, this violation is not being cited
because the criteria in paragraph VII.B.2 of Appendix C to 10 CFR Part 2 of
the NRC's " Rules of Practice," were satisfied.

specified in the enclosed Notice when preparing your raponse.

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results~of future

inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
