r Riverview Medical Center is authorized by NRC License No. 29-09701-02 to possess and use a cobalt-60 source in an. Advanced Medical Systems (Picker)

Model C teletherapy unit.: Aeutron Products, Incorporated is authorized by State of Maryland License No, MD-31-025-03'to service and install sources into these teletherapy units.
Neutron Products is authorized by the general license issued by 10 CFR 150.20 to perform these services in states where NRC maintains regulatory jurisdiction, such as New Jersey.
On April 1,1990, Neutron Products performed a source replacement at Riverview Hospital in Red Bank, New Jersey. A special NRC inspection was conducted to review this source replacement'.
3.
Teletherapy Source Exchange Procedure The replacement source, containing.approximately 6,000 curies-of cobalt-60 was delivered in a shielded Neutron ~ Products shipp.ing cask with three i
drawers for holding sources.
These drawers have a shielding plug at each end to fill the two-inch diameter cylinder which constitutes each 21.5 inch drawer.
The source exchange consists of the followingfoperations.-
1 which were described to the inspector by-the service technicians prior to the exchange:
A.
Source Removal 1.
Preparation of the teletherapy unit for exchange. This operation i
results in the norizontal alignment of.the. teletherapy head.

Removal of both shielding plugs from one of the drawers of the shipping cask.

This includes performance of routine inspection and maintenance of the unit.

The technician wore vendor dosimetry on their wrists and body and also wore self reading dosimeters.
The technicians were aware of the steps-in the procedure which were performed with'the' source partially' unshielded.
The inspector did not observe any departures ~by the service-technicians from-the p'snned source exchange procedures.
4.
Independent Measurements The inspector provided Panasonic Model 801 thermoluminescent dosimeters (TLD) to the' service technicians to supplement the licensee-supplied dosimetry during the source exchange.
During.he source removal procedures, Individual B wore an NRC TLD on the-back or each hand and another TLD on the little finger of his left hand.

During the' source installation, Individual B' wore additional NRC TLD's on the palm of each hand and another NRC TLD on his right thumb.
Furthermore, during the source installation, Individual A wore NRC TLD's at the same locations'as1 Individual B The NRC TLO's were evaluated in the NRC Region-I Dosimetry Laboratory.

Exit Interview 9
The inspector met with the' Neutron Products service technicians at the end I
of the inspection and discussed the scope and findings ofcthe inspection.
The inspector also discussed the inspection by telephone with Marvin Turkanis, Neutron Products Vice President.
