Mid American Inspection Services, Inc.'s program is specific to the use, transpor-tation and shipTent of Special Form Radioactive Sources, Exposure Devices and Source Changers identified on tac Li-cense #21-26060-01.

1.2 Design and/or fabrication of Radioactive Material shipping packages shall not be conducted under this Quality Assur-ance Program.

2.0 ORGANIZATION 2.1 The requirements of this program shall be implemented, using the Organizational Structure detailed on the attadled chart.

3.0 RESPONSIBILITIES 3.1 The Radiaticn Safety Officer shall be responsible for the administration of the program. This shall include train-ing and certification of Canpany personnel, document con-l trol and implementation, i

3.2 The Quality Assurance Manager Shall be responsible for conducting audits of this program to ensure conpliance with the program. The Quality Assurance Manager shall report to the President of the Company.

4.0 QUALITY ASSURANCE PROGRA!4 l

4.1 The Radiation Safety Officer shll implement this Quality Assurance Program. Training for all QA functions, prior to engagement in these functions, is required according to written procedures. QA Program revisions will be made ace-l ording to written procedures with management approval. The QA Program will ensure that all defined QC procedures and l

specific provisions of the package c sign approval are sat-isfied. The QA Program will emphasize control of the char-acteristics of the package which are critical to safety.

This requiremerf. can be satisfied by receiving a certification to this effect fran the manufacturer or supplier of We package.

5.0 DOCUMENT Com'ROL 5.1 All documents related to a specific shipping package will l

be controlled through the use of written procedures. All j

document changes will be performed according to written i

procedures approved by management.

5.2 The Radiation Safety Officer shall ensure that all QA i

functions are conducted in accordance with latest app-licable changes to these documents.

6.0 HANDLING S'IORAGE AND SHIPPING 6.1 Written safety prccedures concerning the handlin;, storage and shipping of packages for certain Special Form Radio-active Material will be followed. Shipment will not be made unless all tests, certifications, acceptances and final inspections have been cxampleted. Work instructions will be provided for handling, storage and shipping opera-tions.

6.2 Radiography personnel shall perfonn the handling, storage and shipping operations.

7.0 INSPECTION, TEST AND OPERATING STATUS 7.1 Inspection, test and operating status of packages for certain Special Fonn Radioactive Material will be indi-cated and controlled by written procedures. Status will be indicated by tag, label, marking or log entry. Status of conforming packag s will be maintained.

l 7.2 Radiography personnel shall perform the regulatory required inspections and tests in accordance with written procedures.

The Radiation Safety Officer shall ensure that these func-tions are performed.

8.0 QUALITY ASSURANCE REODRDS 8.1 Records of package approvals, inspections, tests, operating logs, audit results, personnel training and qualifications and records of shipnents will be maintained. Descriptions of equipment and written procedures will also be maintained.

8.2 These records will be maintained in accordance with written procedures. The records will be identifiable and retriev-able. A list of these records, with their storage locations, will be maintained by the Radiation Safety Officer.

9.0 AUDITS 9.1 Established schedules of audits of the Quality Assurance Program will be performed using written checklists. Results of audits will be maintained and reported to management.

Audit reports will be evaluated and deficient areas corrected.

The audits will be dependent on the safety significance of the activity being audited, but each activity will be audited at least once per year. Audit reports will be maintained as part of the Quality Assurance Records. Members of the audit team shall have no responsibility in the activity being audited.
