SUBJECT:
SPECIAL RADIATION PROTECTION INSPECTION AT LASALLE COUNTY STATION
This refers to the special safety inspection conducted by Mr. P. L. Louden and
Mr. M. A. Kunowski of this office on November 2 through 9, 1993.
The
,
inspection included a review of activities authorized for your LaSalle
facility.
At the conclusion of the inspection, the findings were discussed
with those members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report.
The
l
purpose of the inspection was to review an incident which occurred on
October 31, 1993, while two operators were performing post maintenance valve
,
inspections in the Unit 2 Reactor Water Cleanup Hold Pump Room.
One of the
-
operators received an administrative overexposure of radiation.
During this inspection, certain of your activities appeared to be in violation
l
of NRC requirements, as specified in the enclosed Notice of Violation
(Notice).
These violations are of concern because they illustrate the
,
continued problems with respect to radiological controls previously_ discussed
.
in NRC inspection reports and during an October 5, 1993 Enforcement
!
Conference.
Failure to plan for and recognize the potential for changing
radiological conditions has been a continuing problem at your facility.
i
Additionally, failure to heed electronic dosimeter alarms is a disturbing and
+
potentially significant safety problem. The proper response of radiation
f
workers to alarming dosimeters is an expectation which must be thoroughly
1
understood by radiation workers and enforced by station management.
An
!
environment in which a complacent attitude towards electronic dosimeter alarms -
can exist must not be allowed to occur or be tolerated.
i
We reviewed this event for the possibility that a substantial potential for a
,
regulatory overexposure existed and concluded that given the circumstances of
,
the case it may not have met that threshold. Nevertheless, we have strong
!
concerns about the event because it involved radiological worker performance
problems similar to those discussed in a recent Escalated Enforcement action.

Escalated Enforcement-action dated November 17, 1993.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of. future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
.In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter, the enclosures, and your response to this letter will be placed
in the NRC Public Document Room.
The response directed by this letter and the accompanying Notice are not
subject to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, PL 96-511.
