J. M. Farley Nuclear Plant NRC Inspection April 16 - May 15, 1982 Report Nos. 50-348/82-13 and 50-364/82-1 [

This letter refers to the violation in the subject inspection re-port which states:

"As a result of the inspection conducted on April 16-May 15,1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violation was identified.

Technical Specification 6.8.1.a requires that written pro-cedures shall be established and implemented. Adrinistra-tive Procedure No. 16 " Conduct of Operation - Operations Group" states that plant procedures shall be adhered to in the conduct of all plant operations.

Contrary to the above, on May 10, 1982 plant procedure FNP-1-ARP-1 " Annunciator Response Procedure" for annuncia-tor alarm " Monitor Light Box, Engineered Safety System (ES$)

Valves off-normal position" on the main control board was not utilized to determine which ESS valves were in the off-normal position. This resulted in the centainment spray system being inoperable for six hours and fif ty-five minutes."

Admission or Denial The above violation occurred as described in the subject report.

Reason for Violation The violation was caused by personnel error. The alarm (M.L.B.

  1. 2 ESS Valve Of f Normal) was not investigated properly as required by the Annunciator Response Procedure.

The Containment Spray Suction Valves from the Refueling Water Storage Tank were opened.

A verification of all accessible Containment Spray System valves was conducted. No discrepancies were fcund.

3.

This incident has been discussed with all Licensed Operations per-sonnel. The importance of insuring that the correct switches are actuated when placing equipment in service, removing equipment fro +

service or repositioning valves was emphasized. The importance of recognizing which annunciator alarms are acknowledged and correctly interpreting the information given by the alarmed annunciator and the use of the Annunciator Response Procedure was stressed.

Shift personnel involved were counseled concerning their lack of proper follow up action and apparent lack of attention to detail.

Corrective Steps Taken to Avoid Further Violations See items 3 and 4 above.

Date of Full Compliance All corrective action was completed by May 24, 1982.
