N BELLEFONTE NUCLEAR PLANT UNIT 2 - WELDING OF STRUCTURAL STEEL ANNULUS FLOOR FRAMING - NCR 1173 - FINAL REPORT The subject nonconformance was initially reported to NRC-0IE Inspector R. W. Wright on April 15, 1980, in accordance with 10 CFR 50.55(e). This was followed by our interim reports dated May 15, July 30, and November 7, 1980, February 20, June 23, July 28, and October 26, 1981, and February 9, May 3, and June 7, 1982. Enclosed is our final report.

BELLEFONTE NUCLEAR PLANT UNIT 2 WELDING OF STRUCTURAL STEEL ANNULUS FLOOR FRAMING NCR 1173 10 CFR 50.55(e)
FINAL REPORT Description of Deficiency A TVA welding inspector completed General Specification G-29C visual weld inspection records for welds of structural steel annulus floor framing before the welds had been made. The apparent cause was failure of the inspector to check the weld map versus the welds inspected before completing the inspection records.
Safety Implication Failure to properly inspect the subject welds could have resulted in the acceptance of inferior welds. These inferior welds could have failed which could precipitate the failure of the annulus floor. Safety-related equipment located on the annulus floor and beneath the floor could have been compromised by a floor failure. This condition could have adversely affected the ability to safely shut down the plant.
Corrective Action TVA identified the inspections for which the inspector was responsible and evaluated data compiled from 566 sample welds reinspected per MIL-Standard 105D.
The camples selected were representative of over 4300 visual weld inspections made by the inspector in question for his period of employment, October 1977 to April 1980, at which time the employee resigned. Sampling was performed based upon monthly intervals and the total number of inspections made in that interval for the entire length of employment.
Based upon the results of this investigation, TVA feels that the nonconforming condition identified by NCR 1173 is an isolated case for the individual i
involved. TVA has no concerns that inspections performed by other inspectors are similarly deficient because of our ongoing inspection sampling program.
In support or this conclusion is the following evidence:
1.
Only two welds (OC6-0269 and 0C6-4103) in the total 566 inspected were found not to have been completed, while inspection records indicated that they were. Both of these welds were stiffener to I-beam welds in steel support framing. Similar weld configurations inspected by the inspector in question were reinspected and were found to be completed as the records indicated.
It is reasonably certain that confusion between drawing details and actual as-installed configuration can be attributed to the inspector's oversight in completing these two weld records.
2.
Interviews with craft personnel representing several craft disciplines confirmed that the nonconforming condition was confined to the individual inspector during a specific timeframe in which he was assigned to unit 2 Reactor Building annulus area. Each person interviewed was asked a standard
. set of questions, and the answers were documented and are on file. The scope of the questions included inspectors other than the inspector in question.
TVA will complete repair to those welds nonconformed under the scope of NCR 1173 by August 1, 1982.
