Appendix NOTICE OF VIOLATION Toledo Edison Company Docket No. 50-346 As a result of the inspection conducted on May 6 through June 1,1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982),
the following violation was identified:
Technical Specification 6.8.1 requires the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, November' 1972, to be established, im-plemented and maintained. Appendix A of Regulatory Guide 1.33, November 1972, includes Administrative and Maintenance Procedures. Administrative Procedure-AD 1828.00, " Jumper and Lifted Wire Control Procedure," Section 2.5 states in part that "When installed plant equipment is being removed from the plant by an FCR, jumper and lifted wire tags are required until the equipment, wires, or circuit is removed or labelled as SPARE."
Contrary to the above, on May 18, 1982, cables 1CPR5030C, 1CPR5030B, 1PBE1198A to radiation meter RE 5030 and cables 2PYF211A and 2LRE5029A to radiation meter RE 5029 were not tagged or labelled as SPARE after their disconnection from the radiation meters to allow removal of the radiation meters per FCR 80-050.
This is a Severity Level V Violation (Supplement I)
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or ex-planation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
