APPENDIX A MOTICE OF VIOLATION Arizona rublic Service Company P. O. Box 21666 Phoenix, Arizona 85036 Docket No. 50-529 Construction Permit No. CPPR-142 During an inspection on April 20, 1982, and in accordance with the hhC Enforcement Policy 10 CFR Part 2 Appendix C, 47FR9987 (March 9, 1982), the following violation was identified:
Appendix B of 10 CFR 50, Criterion V, states in part:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings..."
Paragraph 9.5.6 of Ecchtel Specification No. 13-PM-204, Field Fabrication and Installation of Nuclear Piping Systems, requires that if work is interrupted after a pipe spool end closure had been removed, "a temporary closure shall be provided by covering the end with plastic and taping or otherwise sealing".
Contrary to this requirement, on April 20, 1982, after work had been suspended at the end of day shift, five safety related pipe end openings were cbserved to be open in the Unit 2 Auxiliary Building.
This is a Severity Level V Violation.

Pursuant to the provisions of 10 CFR 2.201, Arizona Public Service Company is hereby required to submit to this office within thirty days of the date of this hotice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the.results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
