APPENDIX A NOTICE OF VIOLATION 4
Mississippi Power & Light Docket No. 50-416 Grand Gulf License No. CPPR-118 As a result of the inspection conducted on April 19 - May 14,1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,- 1982), the following violation was identified:
10 CFR 50, Appendix B Criterion V and the accepted QA program 2
(MPL-Topical-1A) Policy 5 requires that activities affecting quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures.
Administrative Procedure (AP) 01-S-06-3, Control of System Alterations, Revision 6, paragraph 6.1 requires documentation of temporary alterations.
Contrary to the above, on May 1,1982 alterations were found in ESF Logic Panel Division I, ESF Logic Panel Division III, in containment penetration pressure switch 1R60PSN010, and on RHR ' A' flow transmitter E12FTN007A which were undocumented. These alterations were made without the documentation required by procedure 01-S-06-3.
This is a Severity Level IV Violation (Supplement II.D.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
