Office of Nuclear Reactor Regulation Items of Interest Week Ending September 17, 1993 Particioation in Industry Occupational ALARA Seminar On September 13-15, 1993, J. Bell, Radiation Protection Branch, participated in an Industry / Vendor technical seminar focused on practical, in-plant engineering and programmatic techniques that have been successful in reducing 3
worker doses at nuclear power plants throughout the world. During the breakout sessions, Mr. Bell answered industry questions on the implementation of the new Part 20.
NUMARC Thermo-Lao Fire Test I. Miller, Plant Systems Branch, observed NUMARC conduct the first of a series of generic Thermo-Lag fire endurance tests at Omega Point Laboratories in San Antonio, TX, on September 9, 1993. This is the first test of Phase 1 of the NUMARC test program and the first three hour endurance test that has been conducted since the NRC started the comprehensive review of Thermo-Lag. The test specimen, a 36-inch wide steel tray straight run with sweeping bends, was enclosed in an enhanced three hour Thermo-Lag barrier. The baseline barrier consisted of three hour panels that were dry-fit and post-buttered. The barrier for one of the sweeping bends used the score-and-fold installation technique with tie wires. The other used the miter technique with bands. The upgrade consisted of an overlay of pre-buttered %-inch panels with wire lacing and stitching of joints. Joints were also reinforced with stress skin.
The barrier exceeded the single point maximum temperature rise (414*F) after 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> and 47 minutes.
Post-fire inspection revealed that the upgrade panel burned away from the bottom of the tray. This allowed a seam to open up and led to a large burn-through on the bottom of the tray. NUMARC representatives present for the test agreed that the test was a failure.

Mr. Correia was a member of a discussion panel on the Maintenance Rule, 10 CFR 50.65. The panel included representatives from NUMARC, Northeast Utilities and Entergy who were involved with the NUMARC verification and validation of the NUMARC maintenance rule implementation guideline. The NUMARC guideline was endorsed by Regulatory Guide 1.160, " Monitoring the Effectiveness of Maintenance at Nuclear Power Plants."
IAEA Technical Committee Meetino During the period of September 6-10, 1993, R. Gallo, Chief, Operator Licensing Branch (HOLB), chaired an IAEA technical comittee meeting relative to nuclear SEPTEMBER 17, 1993 ENCLOSURE A
2 power plant personnel training in Vienna, Austria. The committee recommended that IAEA publish information describing the availability of a proposed training advisory mission and prepared a proposed mission guideline. The committee also recommended that the IAEA pursue proposed measures of effectiveness for the evaluation of facility training programs and developed propos=d measures. Mr. Gallo presented a paper describing the NRC's proposed operator requalification inspection program.
North Anna Power Station Units 1 & 2 (NA-1&2) - Site Visit / Russian Nationals Five representatives of GOSATOMNADZOR (GAN) of Russia visited NA-l&2 on September 16-17, 1993.
GAN is the Russian equivalent of the U.S. NRC. The GAN representatives are participating in a direct assistance priority of the Agency for International Development (AID)-funded Lisbon Initiative of the U.S. State Department. The main purpose of the group's visit to NA-I&2 is to gain a better understanding of how a reactor licensee implements emergency operating procedures (EOPs) and trains reactor operators (R0s). Also, the group received a licensee briefing on the site E0P development and validation process, and observed RO training of E0Ps with the site simulator.
Issuance of Three Mile Island. Unit 2. Possession Only License On September 14, 1993, the NRC staff issued an amendment to license DPR-73, held by General Public Utilities Nuclear Corporation for the Three Mile Island Nuclear Station, Unit 2 (TMI-2). The amendment changes the TMI-2 license from an Operating License to a Possession Only License (POL). Since the March 28, 1979 accident, TMI-2 has held an Operating License, but was prohibited from operating by an NRC order dated July 20, 1979. Under the POL issued on September 14, 1993, the licensee may possess, but not operate, the nuclear station.

Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending September 17, 1993 The Peoole's Reoublic of China Prepares for Implementation of the Nonoroliferation Treaty In response to an invitation from the government of the People's Republic of China (PRC), the Nuclear Regulatory Commission has recently sent a safeguards specialist to present lectures on the NRC's safeguards regulatory programs.
The objective was to assist the Chinese to have a better understanding of the U.S. domestic safeguards system.
PRC has currently in place a basic state material control and accounting (MC&A) system which was patterned after the NRC's MC&A system. However, in the course of preparing for implementation of the Nonproliferation Treaty (NPT), which was signed in September 1992, the Chinese realize that their current system needs to be strengthened.
The lectures were presented at a seminar held in Beijing on August 25 - 28.
There were 35 Chinese government officials and facility representatives invited to the seminar. After the seminar, the NRC representative was invited to visit the China Institute of Atomic Energy and two nuclear installations:
a heavy water research reactor and a power reactor. Technical exchange meetings were held during these site visits. At the conclusion of the China visit, a senior Chinese official indicated that the Chinese government wishes to explore the possibility of further exchanges between NRC and the Chinese nuclear regulatory organizations. The Chinese government has been very satisfied with the positive results of existing agreement between the NRC and the China National Nuclear Safety Administration and wishes to expand the exchange to include safeguards.
Meetino with Siemens Power Corporation - Dry Conversion Exoansion and Criticality Safety Analysis Vodate Prooram On September 8, 1993, representatives of Siemens Power Corporation's Richland Manufacturing Plant gave presentations to the fuel Cycle Safety and Safeguards staff on their criticality safety analysis (CSA) update program and their plans for expansion of their dry conversion facility. The CSA update program is being performed in response to a number of potential criticality events over the past year and has the objective of reviewing and updating CSAs on a plant-system basis. The program is expected to be completed in January 1995.
The dry conversion expansion will increase the dry conversion production capacity of the Richland Manufacturing Facility to 800 metric tons of uranium per year and will allow Siemens to shut down most of the chemical conversion lines and ultimately to close its waste treatment lagoons. Siemens* schedule projects new facility start-up in mid-1996. The amendment application will be submitted in mid-1994.
Meetino with Nuclear Fuel Services. Inc. - Decommissionino Fundino Assurance for the NFS Plant On September 14, 1993, staff members from the Offices of the General Counsel and Nuclear Material Safety and Safeguards met with Nuclear Fuel Services, Inc. (NFS) to discuss decommissioning funding assurance for the NFS plant.

2 The issue arose because NFS indicated their intent to pursue some activities for which the Department of Energy would not guarantee decomissioning funding, and NFS' assurance presented in several recent license amendment i
requests has not been completely in concert with regulatory requirements. The meeting resulted in the resolution of what additional language could be written into the NFS license that would provide adequate assurance that the i
provisions of Title 10, Code of Federal Regulations, Part 70.25, will be met.
The NFS representatives indicated that a license amendment request containing the agreed upon language would be submitted to the NRC in the near future, httelle Memorial Licensino Status During the week of September 13, 1993, a Fuel Cycle Licensing Branch staff member visited the Battelle Memorial Laboratories in Columbus, Ohio, to discuss the status of the their license renewal application and to review progress on site decomissioning activities. With the exception of a few small laboratories for a limited amount of radiology and radioactive tracer studies, Battelle plans to decomission the majority of their contaminated facilities. The major portion of the decontamination and decomissioning will be funded by the Department of Energy; however, there are some facilities where Nuclear Regulatory Comission-licensed activities were carried out and NRC has oversight responsibility.
Issuance of the Final Environmental Impact Statement on Envirocare of Utah On September 13, 1993, the U.S. Nuclear Regulatory Comission staff issued the Final Environmental Impact Statement (FEIS) to Construct and Operate a facility to Receive, Store, and Dispose of Ile.(2) Byproduct Material Near Clive, Utah (NUREG-1476). The issuance of the FEIS completes the environmental portion of the NRC's review of Envirocare of Lah, Inc.
(Envirocare's) application for a license to receive, store, and dispose of byproduct material, as defined in Section 11e.(2) of the Atomic Energy Act.
The safety portion of the review was completed on September 7,1993, with the issuance of Supplement I to the Final Safety Analysis Report (FSER). With completion of the safety and environmental portions of the review, the staff can license the Envirocare facility. We anticipate that the Environmental Protection Agency (EPA) will issue its Notice of Availability (NOA) of the FEIS on September 24, 1993. Therefore, the earliest the license could be issued would be 30 days after EPA's NOA, which would be October 24, 1993.
Reissuance of Proposed Branch Technical Position on Concentration Averaoina and Encapsulation of low-level Waste A notice announcing the reissuance of the Proposed Concentration Averaging and Encapsulation Branch Technical Position is being sent to the Federal &qister for publication. The revised technical position, applicable to the classification of low-level waste, has been modified and significantly expanded in response to coments received on the initial proposal that was noticed on June 30, 1992. The position, together with a sumary of the major coments and staff's responses, is being sent to all Comission licensees, States, those who comented on the original proposal, and others with a request for coment by November 15, 1993. The position, developed with the

cooperation of the E-5 Committee of the Conference of Radiation Control Program Directors, defines a subset of acceptable concentration averaging and encapsulation practices and a process through which other practices may be judged acceptable.
l Shoreham - Exemption from Emeroency Preparedness Reouirements A notice announcing issuance of an Environmental Assessment and Finding of No Significant Impact regarding Exemption of the Shoreham Nuclear Power Station (SNPS), Unit I from the Emergency Preparedness Requirements of 10 CFR 50.54(q) has been forwarded to the Federal Reaister. The exemption grants Long Island Power Authority relief from the requirements to maintain on-site emergency i
response capability. An earlier exemption issued on July 31, 1990, granted relief from off-site emergency preparedness requirements.. The exemption is based on status of decommissioning activities (75% complete), low radiological risk to the off-site population from any credible accident scenario, and existing SNPS plant procedures to address events that may require on-site emergency response.
Industrial and Medical Nuclear Safety Licensee site visits as part of the Quality Management (QM) pilot program were initiated on September 13, 1993. The Office of Nuclear Material Safety and Safeguards and Region I (RI) staff accompanied the contractor (Lawrence Livermore National Laboratory) on five inspections of medical licensees located in RI to evaluate implementation of QM programs previously reviewed by the contractor.
Final site visits as part of the pilot program will be conducted during the week of September 27 in RIII.
Lessons learned as part of the pilot program will be incorporated into the final Standard Review Plan and Inspection Manual procedures.

Office of Nuclear Regulatory Research Items of Interest Week Ending September 17, 1993 Final Workshoo for the International Cooperative Pro.iect on Validation of Geosphere Contaminant Transoort Models The final INTRAVAL Workshop and Coordinating Meeting was hosted by the Swedish Nuclear Power Inspectorate (SKI) in Stockholm, Sweden, August 30-September 3, 1993.
INTRAVAL is an international cooperative study dealing with validation of geosphere contaminant transport models. Approximately 70 scientists and engineers from OECD, NEA and IAEA member countries participated including DOE national laboratories, U.S. Geological Survey, University of Arizona, New Mexico State University and NRC RES and NMSS staff. The workshop focused on lessons learned from the completed model simulations and interpretation of experimental data for the ten test cases covering a range of hydrogeologic media (e.g., fractured tuff) and conditions (e.g., partially saturated).
Significant conclusions were discussed in the context of preparing the final executive summary report. One of these conclusions discussed dealt with model testing and comparison strategies, and the need for designing field experiments that can discriminate among alternative conceptual models.
Another conclusion was the realization of the significant advances in characterization methods and techniques, and numerical methods that developed during the INTRAVAL Project. The sufficiency of presently defined flow and transport processes to adequately explain experimental results was also identified as a significant conclusion. The shortcomings of models to predict long-term and large-scale contaminant behavior beyond those originally defined in the characterization data were also noted. The INTRAVAL reports will document the lessons learned in site characterization and performance assessment related to model validation strategies. Final reports on the test case results, summaries and lessons learned are scheduled for publication in the fall of 1994. The Coordinating Group endorsed a proposal to convene a special session at the Fall 1994 American Geophysical Union Meeting focusing on INTRAVAL lessons learned. Volunteered proposals on post-INTRAVAL cooperative efforts were also discussed (i.e., proposals for the IAEA, OECD/NEA and National Academy of Sciences).

Nicholson, WMB/DRA/RES EXT 492-3856 Dioital Systems Reliability and Nuclear Safety Workshop During September 13-14, 1993, the United States Nuclear Regulatory Commission, assisted by the National Institute of Standards and Technology, sponsored a Digital Systems Reliability and Nuclear Safety Workshop at the Rockville Crowne Plaza Hotel in Rockville, Maryland. The purposes achieved by the workshop were to:
- 1) provide feedback to the NRC from outside experts regarding potential safety issues, proposed regulatory positions, and research associated with application of digital systems in nuclear power plants and
- 2) to continue the in-depth exposure of the NRC staff to digital systems design issues related to nuclear safety by discussions with experts in the state of the art and practice of digital systems.
There were over 250 people in attendance at the workshop. They were from 9 different countries and from 28 states within the United States.

2 Professionally, members of the workshop were from academia, the software community, the nuclear industry, the fossil power industry, the aerospace industry, the Federal Government, and the national laboratories. There were three technical sessions in the workshop consisting of:
- 1) Digital Safety Systems for Nuclear Power Plants, 2) Software Engineering for High Integrity Systems, and 3) Methods for Reducing Risks in Software Engineering. A panel session addressed " Application of Workshop to NRC Activities." Questions and comments from the audience during the panel session resulted in the expressions of many different points of view.
Proceedings from the workshop will be mailed to all registered participants in early 1994.

G Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending September 17, 1993 Diacnostic Evaluation and Incident Investiaation Branch (DEIIB)
On September 17, the Quad Cities Diagnostic Evaluation Team (DET) Manager, along with the AE0D Director, briefed the EDO, NRR Director and Region III Administrator on the team's interim evaluation results. The DET will return to the site the week of September 20 to continue its evaluation.
Incident Response Branch (IRB)
During this week, an IRB staff member, along with a representative from National Oceanic and Atmospheric Administration (NOAA), is participating in a meeting of the World Meteorological Organization (WMO) in Montreal, Canada.
Discussions will concern severe reactor accidents with the potential for transboundary effects.
In particular, procedures are to be drafted concerning the information that the WMD can provide to various countries in the event of a major accident. The combined NRC/NOAA representation will allow for the provision of insights on severe accidents and the information that will be available early in an event to support dispersion calculations.
On September 15, 1993, the DOA Division Director and the Director, Division of Radiation Safety and Safeguards, NRR participated in the Federal Radiological Preparedness Coordinating Committee (FRPCC) meeting. This meeting addressed the Federal Emergency Management Agency's (FEMA) reorganization, the reconstitution of the Subcommittee on Protective Action Guidelines to address emergency worker protective action guides, Federal policy on potassium iodide (KI) for the general public in case of an emergency, and Federal response planning. The FEMA spokesperson reported that James L. Witt, Director of FEMA, has directed the FEMA staff to support the development of an annex to the Federal Response Plan (FRP) to establish the formal linkage between the Federal Radiological Emergency Response Plan (FRERP) and Federal Response Plan. This will clarify the Federal Agency relationships in a Presidentially declared radiological emergency / disaster. This decision will necessitate that modifications to the FRERP and an annex to the FRP be written.
IRB received for review the Preliminary Design Review manual from HFS, Inc.,
the Information Management System Implementation contractor for the new NRC Operations Center in TWFN. This manual specifies the capabilities and equipment which will be used in the new Operations Center.

I A contract was competitively awarded to the University of South Carolina on September 14, 1993, for research entitled "Paleoliquefaction Features on the Atlantic Seaboard." The award is a cost-reimbursement contract in the amount of $99,991 and has a one year period of performance.
Nuclear Reculatory Commission Acouisition Reculation: Minor Amendments A final rule that makes ' minor corrective and conforming amendments to the NRC' acquisition regulation was published in the Federal Register on September 8, 1993 (58 FR 47220). The final rule revises the duration of contract extensions that the Head of the Contracting Activity may approve and identify the Changes Clause as the authority under which changes to the NRC's security regulations are incorporated into a contract. The final rule became effective September 8, 1993.
Maryland Safe Eneroy Coalition: Receiot of a Petition for Rulemakino A notice of receipt of a petition for rulemaking submitted by the Maryland Safe Energy Coalition (PRM-72-1) was published in the Federal Register on September 8, 1993 (58 FR 47222). The petitioner requests that the NRC amend its regulations regarding generic issues related to dry cask storage of spent fuel. The comment period for the petition closes November 22, 1993.

Schiager, et al., (PRM-20-21) was published in the Federal register on September 10, 1993 (58 FR 47676). The petitioners request that the NRC amend its regulations governing the disposal of certain low-level radioactive wastes to provide additional options for the disposal of very low concentrations of short-lived radionuclides. The comment period for the petition closes November 24, 1993.
Notification of Events at Independent Soent Fuel Storace Installations and the Monitored Retrievable Storace Installation A proposed rule that would revise licensee reporting requirements regarding the notification of events related to radiation safety at Independent Spent Fuel Storage Installations and the Monitored Retrievable Storage Installation was published in the Federal Register on September 14, 1993 (58 FR 48004).
The proposed action is intended to ensure that significant occurrences at these licensed facilities are reported to the NRC so that the Commission can evaluate whether the licensee has taken appropriate action to protect public health and safety and whether NRC action is necessary to address generic safety concerns. The comment period for the proposed rule closes November 29, 1993.
Staff Meetinos Open to the Public: Proposed Policy Statement A proposed policy statement that presents the guidance the NRC staff proposes to follow in opening meetings between NRC staff and one or more outside persons to public observation was published in the Federal Register on September 14, 1993 (58 FR 48080). The new OCTel Voice Mail System was accepted by the Nuclear Regulatory Commission (NRC) on September 9,1993. It is fully operational with 1,221 mailboxes created for the NRC staff. As of September 13, 1993, 639 (52.3%) of the mailboxes had been initialized (temporary password changed) by staff members.
All requests for voice mail service received by the Office of Infonnation Resources Management have been satisfied.
New requests for service may be submitted via NRC Form 238.
Division of Contracts ' IBM System /36 Decommissioned On September 10, 1993, the IBM System /36 (S/36) minicomputer and peripherals, that have served the Division of Contracts and Property Management since installation in August 1985, were disconnected when the AUTOS (Agency Upgrade for Technology of Office Systems) Local Area Network (LAN) was implemented for the Office of Administration (ADM).
The S/36 was a node in the IBM 5520 Office Automation Network and primarily supported microcomputers and terminal users accessing the Automated Contracting and IBM DisplayWrite/36 (word processing) systems.

On September 14, 1993, Von Deloatch of the SBCR staff participated in the Tenth Anniversary Celebration of the Science Consortium held by the Department of Energy. The Consortium, which was established in 1983, is a collaborative effort among a predominantly hispanic institution (Ana G. Mendez University System), a historically black college (Jackson State University), and a national laboratory (Lawrence Berkeley Laboratory).
The focus of this year's celebration was on the continuing need for a pool of well-trained minority research scientists who will contribute to the Federal government's science mission. Congressman Solomon P. Ortiz, who was one of the principal speakers, spoke on the " National Imperative for Minority Participation" in all areas of research and science.
OSDBU Directors Group Meetina On September 15, 1993, the Acting Director, SBCR and his program manager attended the Government-wide OSDBU Directors Group Meeting hosted by the U.S.
Department of Justice. The featured guest speaker was Mr. Bruce Gamble, Staff Director for Congressman Kweisi Mfume, Chairman Subcommittee on Minority Enterprise, Finance and Urban Development, Committee on Small Business.
It is expected that the head of every Federal agency will be called in FY '94 to testify what their agency is doing for minority business. The next meeting is schedule for October 14, 1993, at the Office of Federal Procurement Policy, OMB. The topic will be a discussion of the National Performance Review recommendations and its impact on small business.

Office of Enforcement Items of Interest Week Ending September 17, 1993 Sionificant Enforcement Actions A Notice of Violation and Proposed Imposition of Civil Penalty in the amount t
of $10,000, Confirmatory Order Modifying License (Effective Immediately) and l
Demand for Information was issued on September 13, 1993, to Department of Veterans Affairs Medical Center, Birmingham, Alabama. The action was based on an NRC investigation which supports the cited violations of failure to maintain complete and accurate records of at least 100 radiopharmaceutical doses administered to patients during a two year period, failure of the technologist to measure three doses, and failure of the licensee, through the radiation safety officer (RS0), to ensure radiation safety activities were in accordance with regulatory requirements and failure of the RSO to conduct a prompt and adequate investigation. The violations were categorized at Severity Level II. The base civil penalty was escalated 50 percent for NRC identification, 50 percent for poor past performance, and 50 percent for prior opportunity to identify.
In addition, given the duration of the violations, an Order and Demand for Information are being issued requiring an independent audit of the program and assurance that similar violations will not recur.
The staff submitted this action for Commission review in SECY-93-237 and the i
staff was directed to issue the action via SRM dated September 9, 1993.
(EN 93-091) l A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $50,000 was issued on September 14, 1993, to Consumers Power Company (Palisades Nuclear Plant). The action was based on a June 15, 1993 event in which the licensee failed to uncouple one control rod prior to removal of the reactor vessel head. The violation was reflected in (1) the failure of a Shift Supervisor to prebrief the auxiliary operators involved in the control rod uncoupling evolution; (2) failure of the auxiliary operators to conduct a dry run on a mock-up prior to the evolution as required by procedures; (3) the failure to notify the control room after each control rod drive mechanism was uncoupled, as required by procedures; (4) a failure to retain the working copy of the rod uncoupling procedure; and (5) an inadequate procedure for verifying that the rods were uncoupled during removal of the reactor vessel head.
Collectively, these failures represent a breakdown in the controls that are essential for the safe conduct of the vessel head removal activities.
In this case, the base Severity Level III civil penalty of $50,000 could have been mitigated by 50 percent (to $25,000) in recognition of the licensee's comprehensive corrective actions following this event. However, to reflect the level of concern with which the staff viewed the licensee's failure to instill in individuals a proper regard for compliance with procedures and to emphasize the need for careful attention to the control and conduct of the licensed activities involved here, the staff decided to exercise discretion, pursuant to Section VII.A.1 of the Enforcement Policy, and to propose a civil penalty in the amount of the base civil penalty, notwithstanding the application of the mitigation factors. Accordingly, a civil penalty in the amount of $50,000 is being proposed.

A Notice of Violation and Proposed Imposition of Civil Penalty. in the amount of $5000 was issued on September 16, 1993, to Princeton Community Hospital, Princeton, West Virginia. The action was based on two Severity Level III violations that involved the failure to secure radiopharmaceuticals and sealed sources against unauthorized removal and the failure to prepare _ written directives for patient dose administrations of iodine-131 greater than 30 microcuries. The base civil penalty for each of the Severity Level III violations is $2,500. For each violation, escalation of 50 percent was applied for the factor of identification because the violation was identified by the NRC and mitigation of 50 percent was warranted for corrective action.
No further adjustments were warranted.
Eight other violations were cited and categorized at either Severity Level IV or V and involved various administrative requirements related to the radiation safety program.

Civil Penalties Paid Babcock and Wilcox Company, Lynchburg, Virginia, paid the civil penalty in the amount of $37,500. The action was based on a number of violations involving the failure to establish or adhere to nuclear criticality safety (NCS) limits and controls, and the failure to conduct audits and correct audit findings.

Gulf States Utilities (River Bend Station) paid the civil penalty in the amount of $100,000. The action was based on a Severity Level III problem, involving (1) the June 1992 failure to verify post-maintenance internal clearances on a main steam isolation valve (MSIV); (2) the subsequent failure to pursue MSIV closure discrepancies (on the same valve) during February and April 1993 surveillance tests; and (3) the resultant failure to meet Technical Specification requirements for MSIV operability from February 27 to April 18, 1993.

Office of Consolidation Items of Interest Week Ending September 17, 1993 Second Buildina Statn During the week, construction continued on all floors except 2 and 3 and the auditorium and day care areas on Floors P2 and Pl. The excepted floors are awaiting construction permits pending before the Montgomery County construction review authorities. Approval of the day care area is imminent.
Approval of Floors 2 and 3 f s anticipated around September 24. Some slippage in the OMNI schedule for TWFN interior construction completion by mid-November seems likely.
Joann Manzek made changes to space plans for floors 4 and 6 to accommodate a pending Office of Information Resources Management reorganization. On September 13, Ms. Manzek met with Nuclear Materials, Safety and Safeguards (NMSS) staff to complete the review and approval of space plan changes to Floors 7 and 8 resulting from the NMSS reorganization.
Floor 2 plans were modified earlier to reflect the move of the Fitness Center from Floor 2 to Floor 1 and the move of the Office of Small and Disadvantage Business Utilization and Civil Rights from Floor 4 to Floor 2.
On September 15, the TWFN Exhibit Center Committee was formed at an organizing meeting chaired by Beth Hayden of the Office of Public Affairs. Joann Manzek explained the design concept developed by the Office of Consolidation and DBI for the large (1,500 square feet) exhibit space adjacent to the TWFN main lobby.
On September 17, the Director chaired a meeting of representatives of the General Services Administration, the developer (Lerner), the prime contractor (OMNI), the mechanical / electric / plumbing designer (LDE), the interior designer (DBI), and the NRC's Division of Security to discuss issues related to the design and construction of special areas on Floors 4 and 8 requiring high levels of physical security.

Region I Items of Interest Week Ending September 17, 1993 1.
EPA Meetino. Anollo. PA Dr. Walter Pasciak and Mr. Randolph Ragland of the facilities Radiation Protection Section, Division of Radiation Safety and Safeguards, Region I, attended a U.S. Environmental Protection Agency (EPA) meeting in Apollo, Pennsylvania on September 15, 1993. The meeting was held to present the findings of a January 1993 EPA survey of the former Babcock & Wilcox fuel fabrication facility in Apollo to the Apollo Town Council.
Also in attendance were representatives of the Pennsylvania Department of Environmental Resources and the Environmental Protection Agency.

Millstone 3 - Steam Generator 3 Feedwater Nozzle Defects As the result of NRC Information Notice 93-20, the licensee ultrasonically inspected the feedwater (FW) nozzle to pipe joints in steam generators (SG) "B" and "C" and found linear indications in the counterbore area. The defects which were believed to be cracks were subsequently confirmed by RT and MT of the internal pipe and nozzle surfaces. The defects as measured by UT were 0.130" deep in a 0.74"-
1.00" wall 16" diameter piping. They were subsequently removed by grinding and the surfaces of the nozzle were weld overlayed in preparation for rewelding to the piping.
Evaluation of SGs "A" and "D" is still proceeding. These joints, unlike SG "B" and "C," consist of an elbow to nozzle and appeared to show RT indications in the counterbore area (UT was not performed because of configuration). A decision to cut SGs "A" and "D" nozzles is held in abeyance because a metallurgical examination of a pipe section cut from the SG "B" piping did not show the RT indication to be cracking. The licensee is currently planning to cut and inspect SG "A" piping at the SG nozzle, examine the piping internal condition and then make a judgement on SG *D."

LaSalle LaSalle Unit 1 lost offsite power on September 14, 1993, due to failure of the System Auxiliary Transformer and the ensuing reactor scram which resulted in loss of the other source of power to the unit from the main I
generator. The licensee declared an Alert in accordance with its Emergency Plan Implementing Procedures because of the loss of offsite power and the decision to activate the Technical Support Center and the Corporate Emergency Offsite Facility. NRC entered the Monitoring mode and dispatched a Dresden resident inspector to assist at La5.lle. Power to Unit I safety and non-safety buses was established via a cross-tie from Unit 2 which was in a refueling outage. The licensee exited its emergency mode on September 15 when offsite power was restored to Unit 1 by back-feeding through its Unit Auxiliary Transformer. An Augmented Inspection Team arrived at the site on September 16 to review the event and the licensee's response and troubleshooting.

On September 15, 1993, an Enforcement Conference was conducted via telephone between Soil Engineers & Scientists, Inc., and members of the Region III staff. The purpose of the conference was to discuss the licensee's failure to constantly attend a moisture / density gauge.

Wayne County Dept. of Public Services On September 16, 1993, an Enforcement Conference was conducted at Region III between Wayne County Dept. of Public Services and members of the Region III staff. The purpose of the conference was to discuss five i
violations concerning the licensee's failure to meet DOT requirements, failure to have licensed material (moisture / density gauges) under constant surveillance and immediate control, and failure to meet reporting requirements.

Dresden On September 16, 1993, an Enforcement Conference was conducted at Region III between the Regional Administrator and the Dresden Site Vice President. The purpose of the conference was to discuss a potential violation concerning the apparent introduction of a facsimile weapon into the protection area.

Davis Besse On September 16, 1993, the Deputy Regional Administrator and members of the regional and headquarters staff met at the Davis Besse Administrative Building with representatives of Centerior Energy Corporation (parent company of Toledo Edison Company) to conduct a public meeting and present the SALP 10 report. South Texas Pro.iect INPO is conducting a review / assistance visit at the South Texas Project September 13-24, 1993 The team consists of approximately 12 INP0 employees,10 industry peers, and 3 peers from the South Texas Project.

Within these areas, the team is concentrating on equipment performance, human performance, work control, personnel development and training, and leadership.

Comanche Peak Steam Electric Station Meetino and Maintenance Outace Region IV management met on September 16, 1993, with TU Electric officials at the Comanche Peak Steam Electric Station. The licensee presented plans, scheduling, and risk assessment associated with the upcoming third refueling outage of Unit 1.
The outage is scheduled to commence on October 6 and be 56 days in duration.
The licensee also took Unit 2 off line for a maintenance outage. The licensee has been experiencing a feedwater leak inside of containment.
The licensee will repair the leak at the flanged mechanical joint and perform other secondary repairs during a scheduled 2-week outage.
Region IV will increase monitoring of field activities for the outage.
