Box 411 Burlington, Kansas 66839 SUBJECT: NRC INSPECTION REPORT 50-482/93-21 (NOTICE OF VIOLATION) (NOTICE OF ' DEVIATION) This refers to the inspection conducted by a team of NRC personnel on August 9-27, 1993. The inspection included a review of activities authorized for your Wolf Creek Generating Station facility. At the conclusion of the inspection, the findings were discussed with those members of your staff identified in the enclosed report. A supplemental telephone meeting was -. ' conducted on August 31, 1993, to discuss our final determination of one of the issues. , Areas examined during the inspection are identified in the report. Within these areas, the inspection consisted of selective examinations of procedures ' and representative records, interviews with personnel, and observation of activities in progress. The purpose of the inspection was to determine whether activities authorized by the license were conducted safely and in i accordance with NRC requirements. Based on the results of this inspection, certain licensed activities appeared to be in violation of NRC requirements, as specified in the enclosed Notice of - Violation. The first violation is of concern because the implementation of the corrective action program had not progressed as expected considering the ' emphasis you stated it had received. The second and third violations are of concern because they represent failures to follow procedures by operators and
maintenance workers. These instances appear to be further examples of poor . personnel performance that has been documented during other recent NRC inspections. - Based on the results of this inspection, certain of your activities appeared to deviate from your commitments in the Updated Safety Analysis Report, as
specified in the enclosed Notice of Deviation. We are concerned about the deviation because it represents a long-term cond; tion that was not actively pursued to resolution. . You are required to respond to this letter and should follow the instructions

After reviewing your response to these Notices, including your proposed corrective actions and the results of future inspections, the NRC will determine whether further NRC enforcement action is necessary to ensure compliance with NRC regulatory requirements. - - Additionally, we request that you address the corrective action aspects of . these concerns in your response to the procedural violations and the deviation. In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its enclosures will be placed in the NRC Public Document Room. The responses directed by this letter and the enclosed Notices are not subject to the clearance procedures of the Office of Management and Budget as required , by the Paperwork Reduction Act of 1980, Public Law 96-511.
