Dear Mr. DelGeorge:

This refers to the inspection conducted by D. Hills and C. Phillips of this

office, P. Louden and H. Simons of the Division of Radiological Safety and

Safeguards, and by J. Roman of the Illinois Department of Nuclear Safety, on

July 13 through August 27, 1993. The inspection included a review of

authorized activities for your LaSalle County Station, Units 1 and 2,

facility. At the conclusion of the inspection, the findings were discussed

with those members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The first violation was a failure to perform effective

corrective actions. This was of concern because quality control inspectors

were inadequately performing independent inspections of work and management

was unable to promptly correct this problem. The second violation was a '

failure to tape down hoses and cords stretched across contaminated area

boundaries in accordance with station procedures. This was of concern because

of poor radiological housekeeping noticed on several recent plant tours. I

also note that radiological housekeeping concerns have been identified several

times in the past with ineffective long term corrective actions. This matter

needs aggressive management action on your part.

We are also concerned with the lack of planning during the establishment of

your new Operational Support Center. Increased attention is warranted in the

Emergency Preparedness area to ensure that the recent performance decline does

not continue.

You are required by 10 CFR 2.201 to respond to the enclosed Notice of i

Violation. However, you-have taken actions to correct the first violation and ,

prevent recurrence, and those actions were reviewed during this inspection.

Item 2 of the enclosed Notice, you are required to respond to this letter and

should follow the instructions specified in the enclosed Notice when preparing

reviewing your response to this Notice, including your proposed corrective

actions and the results of future inspections, the NRC will determine whether

further NRC enforcement action is necessary to ensure compliance with NRC

regulatory requirements. '
