Box 219-NA-21 Crystal River, FL 34423-0219 Gentlemen: SUBJECT: NOTICE OF VIOLATION (NRC INSPECTION REPORT N0. 50-302/93-18) This refers to the Electrical Distribution System Functional Inspection led by E. Girard of this office on June 28 through July 30, 1993. The inspection included a review of activities authorized for your Crystal River facility. At the conclusion of the inspection, the findings were discussed with those members of your staff identified in the enclosed report. The inspection assessed the capacity of the Electrical Distribution System to perform its intended functions. This was accomplished through a selective review of design calculations, relevant procedures, representative records, and installed equipment; and interviews with engineering and technical support staff. It was concluded that the Electrical Distribution System had the capacity to perform its intended safety functions. However, the reliability of the "B" train safety-related 4160-480 V transformer was questionable. During 1989 and 1990, three of eight similar dry-type 4160-480 V transformers failed at Crystal River. One of the failures was the safety-related "A" train transformer. In 1990 you completed an engineering evaluation that attributed these failures to aging and proposed replacement of the remaining five transformers. Since that time three refueling or midcycle outages have passed and the safety-related "B" train transformer has not been replaced. Please include your schedule for replacement of this transformer in your response to this inspection. Violations of requirements and other findings were identified which require evaluation and resolution. The violations are described in the enclosed Notice of Violation. These and the other findings are described in the executive summary to the enclosed report, with additional details provided in the text. In your response, you should document the specific actions taken and any additional actions you plan to prevent recurrence. 1 After reviewing your response to the Notice, including your proposed corrective actions and the results of future inspections, the NRC will s determine whether further NRC enforcement action is necessary to ensure ' compliance with NRC regulatory requirements. We are requesting that you also respond in writing within 60 days to inspector followup items identified by the inspection, indicating any planned corrective , ' actions and their schedule.
