SUBJECT: NOTICE OF VIOLATION
(NRC INSPECTION REPORT NOS. 50-327/93-33 AND 50-328/93-33)
'
This refers to the routine resident inspection conducted by W. Holland of this
office from July 11 through August 7,1993. The inspection included a review of
activities authorized for your Sequoyah facility.
At the conclusion of the
inspection, the findings were discussed with those members of your staff
identified in the enclosed inspection report.
Areas examined during the inspection are identified in the report. Within these
.
areas, the inspection consisted of selective examinations of procedures and
representative records, interviews with personnel, and observation of activities
in progress.
Based on the results of the inspection, certain of your activities appeared to
violate NRC requirements as described in the enclosed Notice of Violation.
In
,
addition, systems described in the Final Safety Analysis Report appeared to be
not functional as described in the enclosed Notice of Deviation.
We are very
concerned about the safety sensitivity demonstrated at Sequoyah during this
period.
Regulatory
issues
involving
inadequate configuration control,
surveillance performance, design control, and procedural compliance need
continuing management attention. In addition, several examples of " attention to
detail" weaknesses were identified by the NRC which reflect continuing problems
in communication of expectations to lower levels of supervision and craft
personnel.
We will continue to closely monitor personnel sensitivity to
,
regul atory issues during performance of activities in accordance with the
Sequoyah Nuclear Plant Restart Plan.
We did observe that your internal and external assessments identified several
areas for continuing focus. The assessments concluded that positive progress was
i
being made in accordance with your restart plan activities.
We consider your
assessment activities to be a positive indicator of managements willingness to
understand problem areas for implementation of corrective actions.
