(NRC EXAMINATION REPORT N0. 50-369/93-300 AND.50-370/93-300)

This refers to the Nuclear Regulatory Commission (NRC) administered

!

examinations led by Ronald F. Aiello during the period June 28-July 2,1993,

I

and July 12-16, 1993. The examinations were administered to employees of your

l

company who currently hold licenses to operate the McGuire Nuclear Station.

!

At the conclusion of the examination, the examinations and preliminary

findings were discussed with those members of your staff identified in the NRC

'

Examination Report, Enclosure 2.

l

!

A Simulator Facility Report is included in this report as Enclosure 3. . A copy

i

of the written examination questions and answer key, as noted in Enclosure 4,

'

was provided to members of your training staff at the conclusion of the

examination.

,

'

l

l

Based upon the results of the inspection, certain of your activities appeared

!

to be in violation of NRC requirements, as specified in the enclosed Notice of

l

Violation (Enclosure 1). The first violation is of concern because any -

!

'

physical condition that can cause sudden incapacitation is a liability to safe

operation. Similarly, defective hearing or vision is a liability to safe

operation. The second violation is of concern due to the apparent lack of

managerial and administrative controls used to assure safe operation through

'

the use of approved procedures in the control room.

The enclosed examination report identifies activities that appeared to violate

NRC requirements but were not cited. Therefore, no response is required for

,

these items except one. Non-cited violation 50-369, 370/93-300-06, is of

concern because the examiner was unable to determine whether the licensee

!

confirmed the effectiveness of the corrective actions at the completion of

'

requalification remedial training through a suitable evaluation method.

Please include in your response to the enclosed Notice, your corrective

actions for this non-cited violation.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

,

actions you plan to take to prevent recurrence. After reviewing your response

,
