50-410/93-01

This refers to the results of the routine resident safety inspection conducted by Messrs.

W. Schmidt, R. Plasse, and others from January 24, through February 27,1993, at Nine Mile

Point Units I and 2, Scriba, New York. Mr. Schmidt discussed the inspection findings with

members of your staff at the exit meeting conducted on March 9,1993.

The inspectors directed their activities toward areas important to public health and safety. The

enclosed NRC Region I Combined Inspection Report describes the areas examined during the

inspection. Within these areas, the inspection consisted of observation of activities in progress,

'

interviews with personnel, and selective examinations of procedures and representative records.

Also included is a review of Unit I primary containment isolation valve concerns raised in an

October 27,1992,10 CFR 2.206 Petition.

Your staff operated both units safely. This included excellent response by control room

operators to the January 26 reactor scram. However, problems with personnel attention-to-detail

and understanding the impact of actions on the plants during maintenance and surveillance

activities continue as barriers to sustained good performance. Our review of the reactor scram

noted several weaknesses in the conduct of the reactor protection system surveillance test that

directly caused the unit trip. The failure to follow the procedure is a violation; however, based

on the low safety significance and our review of your extensive and aggressive corrective

actions, no response to this violation is required.

Additionally, your staff did not identify an improperly installed Unit 2 Agastat relay until 29

days after the error was made. This was an example of improper work control, in that your

staff did not document the plant impact or post-maintenance test r.spects of the work. This was

compounded by inspector identification that operators did not formally enter a technical

'

specification limiting condition for operation when one train of the low pressure coolant injection

systems was made inoperable during the relay replacement. This issue was unresolved, pending

further review to determine safety significance.
