Moisture Separator Drain ' Dank "A" caused a turbine trip which then caused a reactor trip. '1he high drain tank level was caused by a failed check valve on a line that controls tank level. SWWWhRY: The unit completed its fourth refueling outage on May 15, 1990. Three reactor trips occurred: one near the end of the outage due to operational error and two shortly afterwards due to minor equipment problems. Following these events the unit operated at or near 100Z for the remainder of the conth.
