PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE PN0-II-82-78A This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verifi-cation or evaluation, and is basically all that is known by the Region II staff on this date.

SUBJECT:

OUTAGE EXTENDED This PN0 updates PN0-II-82-78, issued July 8.

The Unit was shut down July 7 to investigate a noise in the "A" steam generator. The licensee reports that inspection revealed that a recently reinstalled tube lane blocking device on the secondary side was loose due to what appears to be improper installation. The licensee concludes that the loose device is the source of the noise.

Similar devices on the "B" and "C" steam generators were inspected and found to be rigid. The attaching devices on all three generators are being strengthened.

During the outage, the licensee is examining thermal sleeves in the reactor coolant system by nondestructive tests. The licensee reports that preliminary analysis indicates that a thermal sleeve in the "B" accumulator line may be missing.

Further examinations are being conducted.

NRR and Region II are reviewing the matter to evaluate the licensee's actions.

Media interest has not occurred. Neither the licensee nor the NRC plans to issue a news release.

The Commonwealth of Virginia has been informed.

Region II (Atlanta) received notification of this occurrence by telephone from the resident inspector at 8:00 a.m. on July 12, 1982.
