r APPENDIX A Lakeside Bridge and Steel Company Docket No. 99900342/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on May 3-7, 1982, it appears that one of your activities was not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantita-tive or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
The Lakeside Bridge 7.nd Steel Company's QA Manual, Revision 6, paragraph 7.2.1.1 states in part, with respect to Manufacturing Process Sheets (MPS), "The MPS shall identify the... applicable procedure and revi-sions...."
Contrary to the above, eight recently completed Manufacturing Process Sheets for Contract No. C-9233 did not identify the applicable revision level for Procedure No. 4.1.2 which was utilized for straightening j
