o APPENDIX A Notice of Violation Texas Utilities Generating Company Dockets:

50-445/82-03 and Comanche Peak Units 1 and 2 50-446/82-03 CPPR-126 and CPPR-127 Based on the results of an NRC inspection conducted during the period of February and March 1982, and in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), 47 FR 9987, dated March 9, 1982, the following violation was identified:

Inadequate Control of Propurement Document _ and Purchased _ Materials Criterion IV of Appendix B to 10 CFR Part 50 requires that necessary requirements be included or referenced to assure adequate quality in procured materials.

Criterion VII further requires that such inspection, as necessary, be conducted by the purchaser at either the source of supply or upon receipt of the purchased material to assure that the specified quality has been attained.

The Texas Utilities Generating Company's, Comanche Peak Steam Electric Station

Quality Assurance Plan implements the requirements of Criterion IV in Section 4.0 by requiring that procedures shall identify the responsibilities and actions required of organizations originating, reviewing, approving, and controlling procurement documents.

The requirements of Criterion VII are implemented in Section 7.0 of the plan by requiring ongoing evaluations of vendors and by requiring receipt inspection of purchased safety-related items.

Contrary to the above, the Senior Resident Inspector (Construction) determined that Texas Utilities Services Purchase Order CP-0701 with Southwest Fabricators, Inc., did not contain or reference adequate quality requirements to assure a satisfactory product nor were the inspections at either the source or upon receipt conducted so as to assure the specified quality was attained.

Specifically:

a.

The referenced Purchase Order states that the materials and/or fabrications will be supplied in compliance with Project Specification 4

2323-MS-43A.

The specification specifically states that it is applicable only to safety-related fabrications in pipe sizes of 8 inches and larger.

During the inspection period, the Senior Resident Inspector observed safety-related pipe fabrications labeled as being furnished by Southwest Fabricators, Inc., that ranged in size from 2 to 6 inches.

b.

The Specification in paragraph 3.7.4.6.1 states that all welds which are to be inservice inspected shall be ground to a flush or flat-topped condition, and that the center of each weld shall be identified by'a marking adjacent to the weld.

The pipe fabrication was identified as RC-2-RB-019-2 in the Unit 2 reactor coolant pressure boundary (Class 1) and was of 6-inch
